APPENDIX A NOTICE OF VIOLATION Long Island Lighting Company Docket No. 50-322 Shoreham Nuclear Power Station License No. CPPR-95 j

As a result of the inspection conducted o August 1 - 31, 1981, and in accordance.

with the' Interim Enforcement Policy, 45 FR 65754 (October 7,1980), the following J

violation was identified:

4 10 CFR 50,. Appendix B, Critarion V states, in part, that:

" Activities Effecting

'uality :; hall be prescribed by documented instructions.... and shall be accomplished 4

in accordance with the instructions...".

FSAR Section 17.2.5 states, in part, that: - The LILCO QA Program established 4

provisions for activities affecting the quality of safety - related structures, i

syste%, and components... to be accomplished 'and controlled in accordance with instructions...".

The LILC0 Operational Quality' Assurance Manual Sectior, 5.3.1 ' states, in.part,ithat:

... activities will be accomplished in accordance with -... documented instructions.

LILC0 Startup Hanual, Section 4.4.2 states, in part, that: "If leads must be -

lifted... the change will be documented in the Lifted Lead 'and ' Jumper Tag-Log".

2.. LILC0 Startup Manual. Section 4.4.2 also states, in part, that: "All tags must have the authort:ation block signed by the Test Engineer".

LILCO,Startup Manual,- Section 4.3.2.3' states, in part, that: "No one.may operate devices with RED tags attached.

LILCO Startup Manual, Section 4.3.4.2, " Procedure for using BLUE tags", states in part: " Remove YELLOW tags if requested as BLUE tags are placed,...

(if not, BLUE tags may not be placed)."

Procedure S.P. 12.035.01, Rev. 2, dated 7/11/79. " Control of Lifted Leads a d Jumpers", Step 8.2 states, in part, that:. "The permit... shall be filled ut with the followed infomation:... Expected duration of the temporary-condition".

Contrary to the above activities affecting quality were not accomplished in accordance with instructions in that on August 5/6,1981:

1.

'The inspector identified the following leads in Panel 601 lifted with no documentation in the Log or Tags hung:

l Leads to pressure indicator E51-PI 001; Two leads from cable E11 BBC 640;.

l Two leads from cable Ell BBC 641; and

-Lead CC 75 to Terminal Board HH.

The authorization block was not signed on tags #1528,1529 and 1838.

The Maine Control Room set of controlled station procedures was not maintained current in that it contained Rev. I vice Rev. 2 of SP 12.035.01, " Control of Lifted Leads and Jumpers".

This is a Severity Level IV Violation (Supplement II.D of the Interim Enforcement Policy).

Pursuant to the provisions of 10 CFR 2.201, Long Island Lighting Company is hereby required to submit to this office within thirty days of the date of this Notice, a writidn statement or explanation in reply, including:- (1) the corrective steps-which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and-(~,3 the date when full compliance will

be achieved. Under the authority of Section 182'of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is -shown, consideration will be given to extending your response time. -
