APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Dockat Hos. 50-277 Peach Bottom Units 2 and 3 50-278 License Nos. DPR-44 DPR-56 As a result of the inspection conducted on July 1 - August 3,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),

the following violations were identified:

A.

Technical Specification 6.8.1 mquires in part: " Written procedures and administrative policies shall be established, implemented and maintained..."

Procedm A-30, " Plant Housekeeping Controls " Revision 4, dated May 21, 1981, states in part:

"a.

Maintenance of Fire Fighting Capabilities:...

The storage of equipment and material shall not impede accessibility to fire fighting equipment...and operability of fire doors. All fire doors and watertight doors shall be maintained closed except when mquired to accommodate the movement of personnel or equipment...."

Contrary to the above:

1.

On July 14,1981. Fire Door No. 228 (on the 135-u it elevation in i

the Radwaste Building) was blocked in the open position; i

2.

On July 28, 1981, Fira Door No.134 Con the 116-foot elevation in the; Turbine Building, to the Laundry) was open; and 3.

On July 31, 1981, Fire Station 135-21 (on ti.e 135-foot elevation in the Unit 2 Reactor Building) was obstmeted by a large portable cart l

and container.

This is a Severity Level V Violation (Supplement I) applicable to DPR-44 and DPR-56.

Technical Specification 6.8, Procedures, states in part:

" Written procedures shall be established, implemented and maintained..." Surve11-lancetestprocedureST2.2.05(Funct),"FunctionalTestofDPIS2-13-84" Revision 10, dated June 8,1981, perfonned July 1,1981 states in part:

" Inform the Reactor Operator that the autoaatic isolation should be reset and the system returned to nomal per procedure 5.3.5.E."

Procedure S.3.5.E., Revision 8. dated May 19, 1981, requires completion of Procedure S.3.5.A to prepare the system for automatic operation. Pmcedure S.3.5. A.,

Revision 7, dated August 5,1980, requires completion of the lineup in accordance with C.O.L.S.3.5.A.

C.0.L.S.3.5.A. " Reactor Core Isolation Cooling System " Revision 10 dated May 30, 1980, specifies "0 PEN" for valve A0-137.

Technica. !;4rc1Mcation 6.11 " Radiation Pmtection Program," states:

" Procedures for personnel radiation protection shall be prepared consistent wIth requirements of 10 CTR Part 20 and shall be approved, meintained and ad5end to for all operations involving personnel radiation exposum."

Health Physics Procedure HP0/CO-5, " Selection and Use of Anti-C Clothing,"

Revision 6, dated April 30, 1980, states ir part: " Anti-Contaminatien clothing requirements for an area are specified by the health physics l

technician and listed on the Radiation Work Permit (RWP)....

If no GP is required, the requimeents are usually posted at the entry poiret, j

If no requirements are posted, contact health physics for entry clothing i

requimments." The procedure requi ms that the individual determine the Anti-Contamination Clothing mquimments from the RWP, area posting, or j

health physics and don the specified clothing.

Contrary to the above on July 13, 1981 an individual failed to adhere to the posted protective clothing mquimsents for the torus entry area,135' elevation of Unit 3 Reactor Building. The posted instruction required coveralls, surgeons cap, two sets of plastic shoecovers, and two pairs of gloves. No other protective clothing requirements were identified by the j

haalth physics technician, and no RWP was in effect.

At the time of the observation, the individual was wearing only one pair of shoecovers and a pair of perforated gloves.

This is a Severity Level VI Violation (Supplement IV) applicable to DPR-56.

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within thirty days of the date of (1)the this Notice, a written statement or explanation in reply, including:) the date j

corrective steps which have been taken and the results achieved; (2 i

when full compliance will be~ achieved. Under the authority of Section 182 of l

the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is shown, consideration will be given to extending your response time.
