APPENDIX A NOTICE OF VIOLATION I
Public Service Company of New Hampshire Docket No. 50-443 j
Seabrook Unit 1 License No. CPPR-135 As a result of the inspection cenducted May 26 - June 26,1981, and in accord-ance with the Interim Enforcemert Policy, 45 FR 66754 (October 7,1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion V states, in part that: " Activities affecting quality shall be prescribad...and shall be accomp(which) shall lished in accordance with... instructions, procedures, or drawings...
include appropriate quantitative or qualitative criteria..."
The Seabrook Station PSAR states, in part, in paragraph 17.1.5 that: "Each organization is required to perfom their respective quality mlated activi-ties covered by this program in accordance with documented instructions, procedures, or drawings (which) shall include appmpriate quantitative or qualitative acceptance criteria.
UE&C Drawing M300228, Sil.C-22 (Revision 2), indicates the typical details for mounting electrical boxes on concrete. Criteria for Hilti anchor spacing am specifically noted, but for anchor location are only pictorially represented.
Contrary to the above, as of June 25, 1981, several electrical boxes in containment mounted in accordance with the noted, but not pictorial, Hilti anchor location requirements of Drawing M300228, SH.C-22, were detemined to be improperly supported in that the loads had been attached to unanchored, cantilever extensions of the strut material.
This is a severity Level V Violation (Supplement II).
Pursurant to the provisions of 10 CFR 2.201, Public Service Company of New Hampshira would be required to submit to this office within twenty-five days of the date of this Notice, a written statement or explaration in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) cornctive steps which will be take.n to avoid further violatior.s; and (3) the date when full compliance will be achieved.
However, your corrective action was taken during this inspection period and has been described and verified in the attached inspection report.
