NOTICE OF VIOLATION Philadelphia Electric Company Philadelphia, Pennsylvania 19101 l
Licen e No. CPPR-106 Docket No. 50-352 This refers to the inspection conducted by a representative of the Region I (Philadelphia) office at the Limerick Generating Station, Unit No. 1, Limerick,. Pennsylvania of activities authorized by NRC License No. CPPR-106.
During this inspection conducted on May 5-8, 1981, the following item of noncompliance was identified.
A.
10 CFR 50, Appendix B, Criterion V, states, in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and... shall include appropriate quantitative or qualitative acceptance criteria..."
Limerick Generating Station, Quality Assurance Plan, Section 5.4,
- states,
" Quality related activities performed by PEC0 personnel are required to be accomplished in accordance with the appendices of this plan and/or written instructions or meedures..."

Quality Assurance Instruction, QAI-10-1, Revision 6, Section 7.4, i
states, "The records described in section 7.1, 7.2 and 7.3 are filed and maintained in the site QA file."
Contrary to the above, Project Special Provisicns Notice, G-5.1, Revision 3 was reviewed, the approval letter transmitted, however, a l
signed' copy of the comment letter was not in the site QA records file l
as required by procedure.

restrary to the above, Project Special Provisions Notice, G-8.1, Revision 1 was reviewed, the approval letter was transmitted, however, the checklist in QAI-10-1 was not signed by the reviewer as required by the procedure.

Quality Insurance Instruction, QAI-5-8, Revision 4, Section 4.2, states, "The members of the Field QA Branch are r2sponsible for reviewing job rules in accordarr.e with this procedure."
Contrary to the above, Job Rule, JR-G-25, Revision 16 was placed in the QA record file although it had not been reviewed.
.This is a Severity Level VI Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
