BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - BOP ISOLATOR CABINET -

BLRD-50-438/81-41, BLRD-50-439/81 REVISED FIRST INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on May 27, 1981 in accordance with 10 CFR 50.55(e) as NCR 1472, This was followed by our first interim report dated June 26, 1981. As discussed with R. V. Crlenjak by telephone on June 30, 1981, enclosed is our revised first interim report. The reason for the revision is to correctly identify the responsible vendor. We expect to submit our next report by September 25, 1981. We consider 10 CFR Part 21 applicable to this deficiency.

REVISED FIRST INTERIM REPORT Description of Deficiency During receiving inspection of balance-of-plant (B0P) wall-mounted isolator

, cabinet 2IX-ISOL-262-G at Bellefonte, TVA employees found ti at a ' conductor 1

was pulled out 'of terminal lug TBA-5A-5. The BOP isolators are used to effectively isolate electrical Class IE from non-Class IS instrumentation.

. The conductor in question is on the . Class IE (input) side of the cabinet.

This condition has not been noticed on any other isolator cabinet. It is apparently caused by interference of the door-locking mechanism with the i

internal wiring. This defect may be ' generic to the other wall-mounted J

isolator cabinets at Bellefonte.

Interim Progress

, The vendor, E. Max, Incorporated, has agreed to examine all wall-mounted cabinets for interference of the locking mechanism with the conductors.

The vendor has agreed to reroute conductors, shield conductors from the

locking mechanism, and/or modify the locking mechanism as necessary to j prevent interference. The vendor will also reterminate the Icose I

conductor.
