Narrative

Report for LER 50-366/1981-057 On 6-23-81 valve 2P41-F315B (Plant Service Water, Division 11 supply to reactor building) was found in the closed position. Valve was immediately opened and equipment served was checked.

No damage or degradation of equipment has resulted from this incident. The event date cannot be determined because the majority of equipment served is not in continuous operation and a review of room and equipment temperatures and alarms have shown no abnormalities. The last date valve is documented as open was on 3-8-81 when normal surveillance (HNP-2-3183) was performed.

Bectuse of the close proximity to door and work having been performed in this area, switch could have been bumped causing valve to operate. A protective cover has been installed over control switches to prevent reoccurrence.
