A Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-456 As a result of the inspection conducted on June 3-5, 1981, and in ac-cordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion XIII, states in part that, " Measures shall be established to control handling, storage, shipping, cleaning and preservation of material and equipment...to prevent damage or deterioration."
The quality assurance Program CE-1-A, Revision 15 for the Braidwood Station under Paragraph 2.2 " Policy" commits to comply with Regula-tory Guide 1.39, Revision 2 which in turn endorses ANSI Standard N45.2.3-1973.
ANSI N45.2.3, " Housekeeping During the Construction Phase of Nuclear Power Plants," states in Paragraph 3.2 that, " control...shall be maintained to prevent the inadvertent inclusion of deleterious materials or objects in critical systems."
Contrary to the above, the primary coolant inlet and outlet of the loop 4 steam generator were uncovered and two worn out paddle wheels together with numerous cigarette butts and reinding dust were found in the plenum. A primary coolant pump casing of loop 4 was also un-covered with papers piled on a suspended seat in the casing.
This is a Severity Level VI violation (Supplement II).
