This refers to the inspection conducted by Messrs. P. Harrell and T. Reis of

this office during the period December 1,1989, through January 15, 1990, of

activities authorized by NRC Operating License DPR-40 for the Fort Calhoun

,

Station, and to the discussion of our findings with you and other members of

l

your staff at the conclusion of the inspection.

Areas examined during the inspection included operational safety verification;

L

plant tours; monthly maintenance, surveillance, security, and radiological

protection observations; in-office review of licensee reports; onsite followup

,

of events; cold weather preparations; housekeeping; and review of the

i

fitness-for-duty program. Within these areas, the inspection consisted of

selective examination of procedures and representative records, interviews with

personnel, and observations by the inspectors. The inspection findings are

documented in the enclosed inspection report.

During this inspection period, an NRC mid-SALP (Systematic Assessment of

Licensee Perfonnance) cycle review of the licensee's performance was conducted.

The results of this review were provided to you and members of your staff by

Region IV and NRC Headquarters personnel during a telephone conference call on

December 6. 1990. The outline of topics discussed are enclosed as Appendix B

to this letter.

During this inspection period, it was found that certain of your activities

were in violation of NRC requirements. However, in accordance with Section V.A

l:

of the NRC's Enforcement Policy, the violation is not being cited. This action

was' taken to encourage and support your self-identification, correction, and

reporting of problems. The noncited violation is discussed in paragraph 12.b

of the enclosed inspection report.

As discussed with Mr. J.- Chase of your staff on February 12, 1990, a management

meeting has been scheduled for March 9,1990, in NRC Headquarters to discuss

the technical issues and significance of the unresolved issues identified in

)

paragraphs 4.a. 5, and 12.b of the enclosed inspection report. These issues

involve the status of the hydrogen purge, raw water, and containment spray

systems and their ability, under certain plant conditions, to fulfill their

intended safety functions.

We have also examined actions you have taken with regard to previously

identified inspection findings. The status of these items is identified in

paragraph 3 of the enclosed report.
