APPENDIX A NOTICE OF VIOLATION Public Service Company of New Hampshire Docket No.

50-443 Seabrook Unit 1 License No. NPF-67 During NRC inspection from December 12, 1989 - January 24, 1990, a violation of NRC Requirements was identified in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR, Part 2, Appendix C.

That violation is, listed below:

10 CFR Part 50, Appendix B requires that activities affecting quality be prescribed by and accomplished in accordance with documented instructions, procedures, or drawings appropriate to the circumstances.

Station Operat-ing procedure 0X 1456.08, "SI/ Charging Pump Monthly Inoperability Check -

Modes 4, 5, 6," requires, in step 8.4, that the safety injection pump supply breakers be racked off the bus, locked and tagged.

The purpose of this measure is to safeguard against primary system overpressure from the safety-injection system.

Contrary to the above, the prescribed safeguard against primary system overpressure was not fully implemented on January 6, 1990, in that the plant was in Mode 5 and.the supply breaker for safety injection pump SI-P-6A was racked and tagged out but was not locked in that position.

This is a Severity Level V Violation (Supplement I).

, Pursuant to 10 CFR Part 2.201, the Public Service Company-of New Hampshire is hereby required to submit to this office, within-30 days of'the date of the letter transmitting this Notice, a written reply including: (1) the reason for the violation, if admitted; (2) the corrective steps which have been taken and the results achieved; (3) the currective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this re-sponse time.

The reply required by this Notice is exempt from the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act l l.
