This letter also serves to close Confirmatory Action Letter (CAL) 86-10, reference (2).

This letter naither imposes any new requirements nor releases Boston Edison from any existing requirements contained in the Facility Operating License or Technical Specifications. The staff finds that management performance, plant materiel condition and operational performance support proceeding with normal operation of the facility.

The bases for approving the completion of the Power Ascension Program and re-lease from CAL 86-10 consist of: (1) successful completion of the Power Ascen-sion Program; (2) a comprehensive assessment of plant and personnel performance by BECo during this phase of plant operation; (3) Final Assessment Report (FAR) submittal by BECo and review of the FAR by the NRC; and (4) NRC staf f inspec-tion and review activities which have confirmed the ef fectiveness of both the staff and management of BECo during this phase of plant operation in assuring safe operation of the facility. is a summary of NRC findings which either have been or will be documented in NRC inspection reports.

However, NRC Staff identified the following three areas in particular from your FM that are key to long-term improvement and will therefore place special emphasis on these areas in our future review of your licensed programs: (1) Pro-cedure Upgrade Programs, (2) Procedure Adherence, and (3) Management Oversight and Critical Self-Assessment. the January 4, 1990 management meeting, the NRC Staff and BECo agreed that the procedure upgrade. schedule in Attachment 2 of the Final Assessment Report will be schedular commitments to the NRC.

.For example, a staff of three resident inspectors and enhanced technical and management attention will-be devoted to the site until the staff has determined

- that NRC oversight can be reduced to that normally devoted to a single unit site.

Additionally, the current status of emergency preparedness for the communities surrounding-the Pilgrim Station, as_ compiled by the staff and reported to,the Commission, is shown in Enclosure 2.

We continue to encourage cooperative and expeditious action.by Boston Edison, the Commonwealth of Massachusetts and local governments to further improve the~ state of' emergency preparedness for the ' area surrounding the Pilgrim facility.

ENCLOSURE 1 ASSESSMENT OF LICENSEE'S COMPLETION AND FINAL ASSESSMENT OF THE p0WER ASCENSION PROGRAM AND READINESS FOR CLOSEOUT OF CONFIRMATORY ACTION LETTER 86-10

Background===

NRC Confirmatory Actio!. Letter (CAL) No. 86-10 of April 12, 1986 and the August 27, 1986 supplement identified specific technical issues to be re-solved prior to facility restart.

In addition, due to other long-term management, hardware and program issues, Boston Edison Company (BECo) sub-mitted a formal power ascension program to the NRC. As specified in the licensee's NRC-accepted Power Ascension Test Program, the program included NRC Regional Administrator approval points at 5%, 25%, 50%, 75% of full power, as well as requiring a licensee formal assessment and NRC review after comoletion of the Power Ascension Program, which includes testing at full power.

By letter dated October 6, 1989, Mr. William T. Russell, Regional Admini-strator for NRC Region I, authorized BECo to proceed beyond the fifth approval point and conduct power operations up to full power. By letter dated D eember 14, 1989, the NRC received the licensee's Final Assessment 9

Report. This enclosure provides (1) the NRC's assessment of the licen-see's performance since the last holdpoint; (2) results of the NRC Restart Staff review of the Final Assessment Report; and (3) results of the NRC Restart Assessment Panel review of the completed Power Ascension Program.

II.

Chronolog, of Significant Plant Events t

October 6, 1989 Licensee received M agreement to proceed from fifth NRC approval point.

October 7 & 12, 1989 On October 7,1989 with reactor power at about 89% and subsequently on October 12, 1989 with reactor power at 100%, the "A" recirculation motor generator (M-G) set tripped when both the motor drive breaker and the generator field _ breaker opened.

In both instances, reactor power stabilized at about 60% following each transient.

The scoop tube on the "A" M-G set remains locked up at this time and the licensee is continu-ing to troubleshoot the M-G set.

October 10, 1989 Pilgrim plant reached 100% power.

2 October 12-13, 1989 The licensee conducted a full scale emergency preparedness exercise on October 12-13,.1989.

Assessment by the NRC determined that the on-site portion of the drill was successful and licensee actions would have protected the public health and safety.

October 13, 1989 Licensee successfully completed second phase (hot standby to cold shutdown) of Shutdown from Outside the Control Room (SDOCR) test.

October 13 -

Licensee successfully conducted a mint-outage November 6, 1989 to perform scheduled maintenance and surveil-lance activities for the plant.

November 6, 1989 The Pilgrim plant was taken critical at 12:08 p.m.

The plant was limited to 94%

power due to inconsistent performance of the "A" recirculation motor generator set speed control mechanism.

December 6, 1989 Fo11gwing a minor modification to the "A" recirculation motor generator (M-G) set speed control circuitry, the M-G set tripped with the reactor power at about 97% when the generator field breater opened due to gene-rator field undervoltage. The reactor power stabilized at about 55% of rated power.

Licensee troubleshooting identified a failed resistor in the M-G set voltage regulation circuitry and a degraded resistor in the M-G set speed control circuitry which might have caused the M-G set trip. After replacement of the resistors and testing of additional components, the "A" recirculation M-G set was returned to service.

December 8, 1989 Automatic reactor scram from about 95% power on a false low reactor water level signal during calibration of a reactor vessel level instrument.

The scram occurred while Instrument & Con-trols technicians were restoring the level instrument to service. When an isolation valve to the instrument rack containing the "A" and "B" reactor level and pressure transmitters was opened, a pressure spike in i

the common variable leg caused the lower reactor water level scram signal. There was no actual low water level condition.

December 11, 1989 The plant was returned to service.

Power reached about 95%'on December 14, 1989, and remained there because the electrical grid load dispatcher requested the plant to defer a power reduction to adjust the control rods to their full power pattern.

December 14, 1989 Test program was completed at the 75% to full power plateau as described in the August 31, 1988 Pilgrim Nuclear Power Station Power Ascension Program.

One operation originally scheduled for the 100% power plateau (the recirculation pump notor generator scoop tube position cleter-minat'on) has not been completed.

This operation will be performed at a later.date.

The licensee revised the Power Ascension Program to reflect this change in accordance with the process set forth in the second Supplement to CAL 86-20.

December 14, 1989 Licensee submittal of Pilgrim Nuclear Power Station Final Assessment Report to NRC.

This report summarizes the results of the self-cssessments and evaluations which Boston Edison has conducted throughout im-piementation of the PNPS Restart Plan and Power Ascension Program.

January 2, 1990 The plant reached full power.

January 4, 1990 The licensee presented results of the Power Ascension Program, with emphasis on the 75%

to full power plateau to the NRC Pilgrim Restart Assessment Panel. This presentation was at a management meeting which was open to the public.

III. Staff Evaluation 1.

The NRC Restart Assessment Panel considered the following in evaluat-ing the conduct of the Pilgrim Power Ascension Test Program and con-sidering approval of normal operation of the facility:

Staff assessment, updated through January 15, 1990, of licensee per-formance during the 754 to full power plateau of the Power Ascension Test Program.

a.

ElantStatusandEquipmentOperability The licensee has determined and the NRC is satisfied that equip-ment required for normal full power operations is operable, When equipment inoperabilities have occurred, the licensee has handled them conservatively and within Technical Specification requi rement;,.

One equipment problem ideatified during the Power Ascension Test Program remains unresolved: the tripping of the "A" recirculation pump motor generator (M-G) while in manual-remote mode at high flow conditions. On October 7,1989 with reactor power at about 89%, on October 12,1989 with reactor at 100% and subsequently on December 6, 1989 with reactor at about 97%, the M-G set tripped when the generator field breaker opened due to generator field undervoltage. Each time, reactor power stabilized at about 55% to 60% of rated power following the transient.

Licensee investigation and trousleshooting have failed to identify a conclusive reason for these M-G set trips.

The "A" Recirculation M-G set scoop tube is " locked up" at this time.

On January 11, 1990, one of the five deep-draft salt service water pumps was dropped about one foot to the ground after re-moval from the systein as it was being positioned for mainten-ance.

The drop caused brittle fracture of one of the pump's five eight-foot shaft casing columns. Based on the design mate-rial properties on the column, it should have failed in a duc-tile manner, if at all. The licensee is conducting an expedited evaluation of this event and also plans to inspect the other four pumps once the fifth is repaired. The licensee is confi-dent that the stress of the drop was significantly higher than any combination of design stresses for an installed pump and is completing confirmatory calculations. However, the licensee has indicated that, should one of the four in-service pumps fail and a similar column failure cannot be ruled out as the cause, the

i plant will be promptly shut down (i.e., within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />).

Nor-4 mally, a seven-day Technical Specification Action Statement would apply. NRC Region I is-closely following licensee activi-ties.

l Required non-safety-related equipment is in a condition that supports safe plant operation.

Surveillance tests are up-to-date with no significant identified discrepancies. The licen-see's' management review of facility cperation up to full power operation is complete. Operation of the facility was conducted in a safe manner and concerns raised by the NRC and BEco manage-ment regarding facility operations were appropriately addressed.

In summary, the NRC Restart Assessment Panel is satisfied that r

equipment required for continuation of normal full power opera-tion is operable.

Management / Personnel Performance and Oversight Activities The NRC Restart Staff has continued to monitor management and personnel performance since start-up.

BECo management has been generally responsive to tiRC concerns and has acted in a cautious, deliberate manner.

Licensee peer evaluators were observed and have been providing good feedback to plant personnel. Operators were generally knowledgeable of their duties and plant condi-tions and management maintains an active and effective oversight of operations.

Operators responded very well to events that occurred during this phase of the PATP.

These events included three recircula-tion pump motor generator trips and one reactor scram.

The Restart Inspection Staff observed Phase II of the licensee's Shutdown from Outside the Control Room (SDOCR) test. The 11cen-see's preparation, communications, and command and control were excellent.

Test personnel were aware of their responsibilities, skilled in their assignments, and demonstrated excellent plant knowledge, quick thinking and ef fective teamwork.

From preparation through execution, this test.was performed in a safe and satisfactory manner.

Phases I and II of the Shutdown from Outside Control Room Demonstration were two cases of exem-plary performances by the licensee's operations department dur-ing the PATP.

During the 75% to 100% power plateau, management performed in a generally cautious, deliberate manner. Management was observed

s involved in decision-making.

of events continue to be comprehensive and properly focused, i

demonstrating a safety-conscious attitude.

From October 13 through November 6, 1989, the licensee conducted a mini-outage in order to perform scheduled maintenance and sur-veillance activities. The NRC Restart Staff determined that i

overall, the October mint-outage was conducted in an extremely professional, well-organized and conservative manner.

Particu-r larly noteworthy were the conservative management decisions made at several junctures during the outage. Operations staff cogni-rance of maintenance activities during the outage was superior 4

to that observed during the preceding nine months of the PATP.

During this assessment period, the NRC Restart Staff audited selected tagout documents generated during the October mainten-ance outage.

The audit consisted of about fifty completed tag-outs and five active tagouts.

Review of the tagouts revealed that over 80% of the tagouts contained some error or omission, t

Over 30% of the reviewed tagouts had significant errors such as tags hung or cleared by individuals other than those specified on the tagout and, in one case, tags cleared without Nuclear

Watch Engineer signature, In a sampling of approximately ten tagouts involving work which required the lifting of leads or the installation of jumpers, over 50% of the jobs were not entered in the Lifted Lead and Jumper Log.

Four of the in-stances noted involved the replacement of safety-related com-ponents (e.g. Reactor Level Transmitter; Diesel Generator Emer-gency Start Relay).

The licensee confirmed the NRC findings, identified possible causes and has detailed a corrective action and prevention pro-gram.

Included as part of this program is the promulgation of a new procedure requiring periodic tag audits and a'one-time Qual-ity Assurance tagging audit to be completed early in 1990.

should be noted that no hardware problems were identified (e.g.

tags were hung on the correct valves).

Licensee follow-up of this issue has been appropriate.

These findings confirm previous NRC assessments that have con-cluded that, while performance is acceptable and continues to improve, the areas of procedural adherence and attention-to-detail require continued licensee management attention and NRC monitoring.

Power Ascension Testing Program Adequacy The licensee's Power Ascension Test Program was completed in accordance with the licensee's commitments documented in the December 30, 1988 supplement to Confirmatory Action Letter

86-10. No significant problems or technical issues were out-1 standing as a result of implementing this program.

Ascension Program to reflect this change in accordance with the l

process set forth in the second Supplement to CAL 86-10. The Staff has reviewed this change during routine inspection and found it acceptable.

The licensee successfully completed the second phase of the Shutdown from Outside the Control Room Demonstration on October 13, 1989. The first phase of the SDOCR demonstration had been successfully completed on June 29, 1989.

Power Ascension Test Program Results The NRC is satisfied that tests and conditions required to be completed by the licensee during performance of the NRC-accepted Power Ascension Program up to full power have been completed.

Satisfactory results were achieved during the performance of:

(1) Reactor Engineering Tests; (2) Drywell Temperature Profile Measurements; (3) Shutdown Outside the Control Room Demonstration (Phase II).

Also, the NRC evaluated licensee management and plant staff per-formance during planned periods of normal operations at about full power and during the maintenance mini-outage. During this-period, licensee management had, as planned, reduced the level of operations oversight. The NRC panel has concluded that plant operations are being conducted safely and conservatively.

The NRC has reviewed BECo's actions in response to inspection findings which had potential to impact further reactor opera-tions and found no outstanding issues which would preclude the licensee from proceeding with normal full power operations, e.

NRC Inspection Program Status With the acceptance of the licensee's completion and final assessment of the Power Ascension Program and close out of Con-firmatory Action Letter 86-10, the NRC Staff will end its

special NRC Restart Staff inspection program and NRC Restart Assessment Panel oversight of Pilgrim. However, NRC attention to Pilgrim will remain above that typical of a single unit site.

A staff-of three resident inspectors and enhanced Region I and NRR technical and management attention will be devoted to the site.

There are no open allegations at Pilgrim affecting a recommenda-i tien to approve continued normal full power plant operation.

There are no enforcement actions pending which would preclude continued plant operations. NRC Region I Inspection Report 50-293/89-12 details one violation (Level IV) concerning devi-ations from the procedural requirements of the newly revised tagout procedure. However, the Panel has determined that in-itial short-term corrective actions for this issue are suffi-cient.

Therefore, the panel concludes that there are no pending i

enforcement issues which would preclude operations up to full j

power.

3.

Review of Licensee Completion of Power Ascension Test Program (PATP),

Performance of a Final pATP Self-Assessment, and Satisfaction of Confirmatory Action Letter (CAL) 86-10 t

The Staff also performed a retrospective overall. review and evalu-ation to assess whether the licensee has successfully and effectively completed the Power Ascension Test Program, has performed a meaning-ful and critical self-assessment and is making appropriate short-term and long-term use of lessons learned.

In the course of this review, the staff placed special emphasis on assessing: (1) the licensee's plans and measures for transition from the Power Ascension Program to normal operation, (2) the licensee's measures for incorporating lessons learned into permanent programs, and (3) the licensee's plans for continued self-evaluation, self-assessment, and self-improvement.

l During its deliberations, the Staff evaluated whether the licensee had fully satisfied CAL 86-10 and whether the CAL should be closed i

out,

3.1 Background

The Second Supplement to Confirmatory Action Letter (CAL) 86-10 was issued December 30, 1988 as a letter from William T. Russell (NRC Region I) to R. G. Bird (BEco). This supplement indicated that CAL i

l-conditions had been satisfied up to the point of carrying out plant restart and power ascension, and that successful implementation of the Power Ascension Test Program (PATP) was necessary for final closeout of the CAL. The supplement further indicated that following completion of the PATP and licensee submittal of a report, the staff would form its independent conclusion regarding program effectiveness

I and would consider final closure of CAL 86-10. The licensee con-cluded that the PATP was successfully completed and submitted their Final Assessment Report (FAR) of the PATP on December 14, 1989.

3.2 Review of Final Assessment Report As part of its review of PATP completion and program effectiveness, i

the NRC staff reviewed and evaluated the licensee's FAR.

The. staff's review was coordinated through the NRC Pilgrim Restart Assessment Panel and included the following:

review of the FAR by Panel members and other staff; l

discussions with licensee personnel; selected review of FAR back-up material and supporting documents maintained by the licensee; comparison of licensee evaluation and cnnelusions to NRC inspec-tion and review findings; and a management meeting with the licensee on January 4,1990.

In addition, the staff conducted a small number of confirmatory in-spection activities directly related to FAR issues.

These will be reported in NRC Inspection Regort 50-293/89-13. The need for these confirmatory inspections after NRC receipt of the FAR was minimal in view of the extraordinary inspection efforts that had been applied throughout the PATP.

The staff concluded that the FAR represents an acceptable self-assessment and indicates successful completion of the PATP. The con-tent of the FAR is summary rather than detailed.

However, it is well-supported by backup documentation maintained by the licensee.

The licensee managers are knowledgeable and well-versed in.the issues involved -- they were able to provide detailed answers to staff ques-E.

tions during the management meeting and other staff questioning dur-ing the PATP. The licensee FAR provides a slightly more positive l'

view of overall performance than NRC inspection documents; however, s

the difference is not significant. The licensee's final assessment agrees closely with the staff's assessment regarding safety signi-ficant areas to be targeted for further improvement and continued management attention. The FAR also identifies needs for streamlining I'

administrative processes for the sake of efficiency; licensee plans for approaching these areas include measures to preclude degradations in the level of safety assurance. This will be evaluated in the course of future routine inspections as the licensee develops speci-fic changes.

The FAR provides a set of integrated lessons learned and summarizes licensee measures to sustain performance improvements through per-manent and long-term programs.

The staff concludes that the licensee

has appropriately identified measures for sustaining improvement.

The NRC Staff identified the following three areas in particular that are key.to long-term improvement and will-therefore place special emphasis on these areas in our future reviews:

Procedure Upgrade Programs; Procedure Adherence; and Management Oversight and Critical Self-Assessment..

With regard to Procedure Upgrade Programs, at the January 4, 1990 management meeting the NRC staff and the licensee agreed that the procedure upgrade schedule estimates in Attachment 2 of the FAR are BECo schedular commitments to NRC.

3.3 Power Ascension Test program Completion Previous NRC assessments have documented satisfautory licensee com-pletion of the PATP up to the J5% approval point.

Staff inspection and review activities have now concluded that the licensee has com-pleted the PATP through full power, including plant power ascension, testing, periods of normal operation, and a management assessment.

Whe're the licensee has made changes to the details of the program, they have met the NRC staff stipulations in the CAL 86-10 Second Supplement, dated December 30, 1988.

NRC inspection activities re-lated to the final phase of the PATP, operation to full power, will be documented in NRC Inspection Reports 50-293/89-12 and 89-13.

Based on these reviews and on staff review of the FAR described in paragraph 3.2 above, the staff concludes that the licensee has suc-4 cessfully completed the PATP.

3.4 Licensee Satisfaction of CAL 86-10 The CAL 86-10 Second Supplement indicated that successful completion of the PATP was necessary for final closeout of CAL 86-10. As dis-cussed above, the licensee has successfully completed the PATP and submitted an acceptable report. Although performance lapses and weaknesses have occurred, the extensive licensee efforts since origi-j nel issuance of CAL 86-10 have resulted in significantly improved safety performance when compared to historical performance prior to the CAL.

The NRC Restart Assessment Panel met on January 4,1990 to review the lic-4 ensee's completion of the Power Ascension Program and the Final Assessment Report.

Boston Edison Company (BEco) had completed the Power Ascension Program and submitted their Final Assessment Report of the Power Ascension Program on December 14, 1989.

The NRC Staff reviewed the licensee's Final Assessment Report and concluded that the Final Assessment Report repre-sents an acceptable self-assessment and indicates successful completion of the Power Ascension Program.

In addition, Staff inspection and review activities have nos concluded that the licensee has completed the Power Ascension Program.

The Confirmatory Action Letter 86-10 Second Supplement i

indicated that successful completion of the Power Ascension Program was necessary for final close out of CAL 05-10. Based on the above, the Staff concludes that CAL 86-10 may be closed.

The NRC also concludes that, with the completion of the Power Ascension Program, submittal of the Final Assessment Report and. completion of the conditions of CAL 86-10, the work of the NRC Restart Staff and NRC Restart Assessment Panel is complete.

However, NRC attention to Pilgrim will re-main above that typical of a single unit site.

ENCLOSURE 2 Current Status of Emergency Preparedness for the Communities Surrounding the Pilgrim Station Progress continues to be noted in the state of emergency preparedness for Pilgrim in the successful conduct of the full participation exercise on October 12-13, 1989, completion of renovations for the Massachusetts Civil Defense Agency (MCDA) Area 11 Emergency Operations Center (EOC) and Wellesley Reception Center, continued resolution of Reception Center issues, and the continued active and visible involvement in the review process by the Fede.al Emergency Management Agency (FEMA).

The full. participation exercise was conducted on October 12-13, 1989. Each of the J

towns, ie., Carver, Duxbury, Kingston, Marshfield, Plymouth, Bridgewater and Taunton fully participated. Additionally the Commonwealth of Massachusetts Civil Defense Agency, Massachusetts State Police, Massachusetts Department of Public Health, various State Agencies (with representatives in the EOC), selected National Guard units, and Plymouth County fully participated. FEMA observed all activities.

Preliminary oral results presented by FEMA at a public meeting conducted on October 16,1989 indicated that no major problem areas were identified. A deficiency report was issued by FEMA on February 1,1990 and indicates deficiencies against Massachusetts regarding the timing and content of Emergency Broadcast System (EBS) messages and command and control of the Massachusetts EOC. FEMA will require that Massach6setts demonstrate thes areas in a remedial exercise within about 60 days.

FEMA has given February 28,1990 as the current schedule to issue the draft exercise report.

In Plymouth,91 of 92 implementing procedures have been approved in concept by the Board of Selectmen and all have been forwarded to MCDA for review. Plymouth has requested technical assistance from MCDA regarding the school procedure. All implementing procedures except for the school procedure have been forwarded to FEMA for technical review. FEMA has not completed its review.

In Marshfield, the town is considering naming a host school within the town borders, but greater than 15 miles from the Emergency Planning Zone. MCDA opposes this action. A meeting is scheduled between Marshfield, MCDA and BECO planners on i

Each town provided sufficient staff to conduct the exercise. Personnel to staff those vacancies that still exist are being actively pursued.

Each town completed sufficient training to adequately prepare and participate in the exercise. Refresher training will be conducted in 1990. This training will be the similar that provided in 1989, but will capture revisions made to resolve areas identified as needing corrective action as a result of the exercise. The revisions and resultant lesson plan revisions are dependent upon the issuance of the FEMA exercise report.

Progress continues on the reception centers. In Taunton, the reception center is fully operational and was fully demonstrated during the exercise. Renovations to enhance operations are in progress and are scheduled to be completed by April 1,1990. In Bridgewater, all equipment is in the town and available for use at the reception center.

Staffing is compSte except for Monitoring and Decontamination Staff. Bridgewater has officially requested MCDA assistance in this area as they can not support that function.

MCDA anticipates utilizing a local National Guard unit for this function. Renovations at the Bridgewater State College have commenced, but are presently on hold pending resolution of the Monitoring and Decontamination staffing problem. Bridgewater conducted a walk through of the reception center during the exercise. In Wellesley, renovations are essentially complete for the EOC and decontamination facilitie's. All equipment is stored at the facility. Red Cross has identified mass care facilities and is scheduled to conduct a detailed shelter survey. Parking is available onsite as well as at the Sun Life company and the Massachusetts Bay Community College. Transportation has been arranged for shuttle service from the remote parking areas. All areas are essentially fully staffed. The Emergency Plan and 20 of 21 Implementing Procedures have been drafted. MCDA has tentatively given June 1,1990 as the date the reception center will be fully functional, including completion of renovations, planning, staffing and training.

A Regional Assistance Committee meeting was held on October 3,1989 at which Commonwealth and MCDA Area 11 emergency plans and implementing procedures were reviewed.

The attached emergency preparedness summaries provide the January 5,1990 status of selected emergency preparedness issues for each town surrounding the Pilgrim Station, the Commonwealth and MCDA Area II as compared to that of September 8,1989.

The staff provided the summaries to the respective town officials through the Civil Defense Directors of each area and subsequently contacted each official to verify the accuracy of the summaries.
