This refers to the team inspection conducted by members of our staff from

.0ctober 23 through Nosember 9,1989, of activities authorized by NRC Operating

'

,

License DPR-40-for the. Fort'Calhoun Station. Our findings were discussed with

you and other members of your staff at the conclusion of the inspection.

,

,

' Areas examined during this inspection-included systems selected on the basis of

r

relative risk from the generic accident sequences that were developed by a

'

generic based probabilistic risk ass, ssment. During the equipment inspection,

,

the inspectors focused on the availability of critical components as evidenced-

>

,by plant conditions and implementation of programs and procedures that would

,

i

ensure operability. Within these areas, the. inspectors examined selective

procedures and representative records, interviewed personnel, and observed

equipment and-simulated activities.

During this inspection, the. inspectors- found that some of your activities were

in violation ~of NRC requirements. Consequently, you are required to respond to

this violation, in writing, in accordance with the-provisions of Section 2.201

of the NRC's " Rules of Practice," Part 2, Title 10, code of Federal Regulations,

Your response should be based on the specifics contained in the: Notice of

Violation enclosed with this letter.

The inspectors also identified another activity that was in violation of NRC

requirements. However, in-accordance with Section V.A of the'NRC's' Enforcement

Policy'(10 CFR Part 2, Appendix C), no citation is being issued for that

,

activity. The violation is described in_Section 4 of the enclosed inspection

report.

In addition, during the' exit meeting for this inspection,

Mr.

S..~.K. Gambhir of your staff made a commitment relative to weaknesses

. identified in your fuse control program. This commitment involved taking

' interim corrective actions within 5 days of the exit by having-a short-term

fuse control program in place and having a comprehensive fuse control program

-developed by the end of the spring 1990 refueling outage.

If these actions are

Tnot consistent with your understanding of the commitment made please notify us

within-30 days of the date of this letter.

This inspection identified many problems with Emergency Operating

'

Procedures (EOPs); these are discussed in paragraph 2 of the attached report.

Although the inspection concluded that your E0Ps, when used by experienced

'

. operators, provided adequate direction to mitigate accident consequences, the

number of problems identified in a small sample of E0Ps is cause for concern.-

'

A complete E0P inspection is planned for later this. year; however, we

recommend that corrective actions for your E0Ps not be held in abeyance,until

this E0P inspection.

The responses directed by this>1etter and the! accompanying Notices are not

subject to the clearance procedures of the'0fficeiof Hanagement and Budget as'

required by the' Paperwork' Reduction Act of 1989,'PL96-511.
