NRC Region I Inspection Report No. 50-293/89-12
This refers to the inspection conducted by the NRC Restart Staff on October 2
through November 19, 1989 at the Pilgrim Nuclear Power Station, Plymouth,-
Massachusetts. Areas examined during this inspection are described in the NRC
'
Region I Inspection Report which is enclosed with this letter.
The Restart Staff, through selected around-the-clock shift inspections and
routine program inspections, observed management controls, conduct of opera-
tions and startup testing ectivities of the licensee Power Ascension Test Pro-
gram, including licensee performance during the October maintenance mini-
outage.
Our Restart Staff determined that overall, the scheduled October maintenance
outage was conducted in a very professional, well organized and conservative
manner.
Particularly noteworthy were the conservative management decisions
made at several junctures during the outage. This item is discussed-in detail
in Section 8.0 of the enclosed report.
However, it appears that one of your activities was not conducted in accordance
with NRC requirements as set forth in the Notice of Violation, Appendix A, en-
closed herein. The violation has been categorized by severity level in accord-
ance with the NRC Enforcement Policy (10 CFR 2, Appendix C).
You are required
to respond to this letter and -in preparing your response, you ~ should follow the
instructions in Appendix A.
The violation involves failure to follow procedures associated with tagouts.
Previous failures to follow these procedures were the subject of NRC enforce-
ment action subsequent to the April 1989 RCIC system suction overpressurization
event. Even though the recent failures to follow procedures are different in
nature and of substantially lower safety significance than those of April 1989,
this area nonetheless warrants your management attention to ensure effective-
ness of corrective actions to preclude recurrence.
The response directed by this letter and the accompanying notice are not sub-
ject to the clearance procedures of the Office of Management and Budget as re-
quired by the Paperwork Reduction Act of 1980, PL 96-511.
