Seasoyah. Fuels. Corporation.Reorosoization On December 29. 1989, SequoyahFuelsCorporation(SFC)wasauthorizedto l'

reorganize and create _a,new corporate entity, New Sequoyah Fuels Corporation (NSFC), and to establish NSFC as a wholly-owned subsidiary of SFC. The

- t operating facility, the. decommissioning reserve account, and other assets, excluding the ranches and existing assignment contracts, have been transferred from SFC to NSFC.

Chio Governor. Approves Umbrella.MOU.with..NRC Region III has received word from the State Liaison Officer of the State of Ohio that Governor Richard Celeste wishes to proceed with signing an umbrella

- memorandum of understanding (MOU) with the NRC. This would be the first M0V to

' be agreso to with a State under the Comn.ission's policy on cooperation with

-States of February 22, 1989. We expect that the State will-wish to pursue detailed subagreements subsequent to the umbrella MOU. A Comission paper with a proposed MOU is being prepared for Comission approval.

US-USSR. COOPERATION o

Mr. -James Taylor EDO and U.S. Co-Chairman of the Joint Coordinating Comission on Civilian Nuclear Reactor Safety (JCCCNRS), had the opportunity on January 4, 1990 to_have a-morning meeting with Dr. Nikolai Ponomarev-Stepnoy, the Soviet Co-Chairman of the JCCCNRS. Also present it the meetings were other NRC members of the JCCCNRS, Mr. Denton, Mr. Shea, Mr. Shomaker, Dr. Speis, Dr.

l-p Ross, Mr. Jordan, and Mr. Reynolds from Department of Energy and Fr;nk Kinne11y

'from the Department of State. This was an opportunity for further coordination 7

on working groups and activities planned for 1990 and all parties expressed E

positive coments about our continued US/ USSR Cooperation.

ColdShutdown-ExtendedRefuelingOutage(coreoff-loaded). The licensee expects to load fuel. in January,1990 and to be ready for the STARTUP. mode f

of operation 54 days later, Niagara Mohawk has continued to postpone the planned reloading of the

-Unit 1 fuel. ' Maintenance activities, surveillance testing, and reviews of paperwork had to be completed, and day-to-day delays in the beginning of reloading have resulted. Niagara Mohawk's decision to not reload during the Christmas and New Year's weekends has also delayed progress. On 12/28/89, work was underway _ to replace a control rod drive mechanism which could not be coupled to the control rod. However, reloading of the fuel is imminent.

Unit;2:

On December 26, 1989, Unit 2 shutdown to hot standby to repair a cracked weld.on the "A" feedwater (FW) pump and a seal failure on the "C" FW pump.

Vibration problems have been experienced on the "B" FW pump. As of 01/03/90, the A pump is operable, the B and C pumps are disassembled for Linspection and repair, if necessary, of the impe11ers. Restart is targeted for 01/15/90.

Niagara Mohawk announced a management reorganization to provide a deputy station superintendent at Unit 2 and to fill this positior, with the previous Unit 2 operations superintendent. The previous manager of-regulatory compliance will become the Unit 2 operations superintendent.

Rick. Abbott will continue to be the Unit 2 station superintendent.

YANKEE R0WE' Yankee submitted their response to Generic Letter 88-20, " Severe Accident Closure Submittal for Yankee Nuclear Power Station." That submittal is the first of any licensee responses to Generic Letter 88-20 and is scheduled to be first reviewed. -That review is to be a joint Research, NRR and Contractor effort and its completion has been promised by mid-year.

Its completion bears on Yankee's' Plant Life Extension.

LInternational Meeting with Department.of. State.on. International. Atomic Energy. Agency-(I AEA)

Inspections On-January 3,1990, a representative from the Office on Nuclear Technology and Safeguards of the Department of State met with staff members of the -

International Assistance Section, NMSS, to discuss practices of the IAEA during inspections at fuel facilities. Some technical' questions regarding IAEA sampling and measurement practices were answered by the NMSS staff.

NRC.DCW Program Workshop A workshop was held in Annapolis, Maryland, on December 18-19, 1989, to examine in detail the NRC Direct Containment Heating-(DCH) Program to determine whether experimental tasks as currently defined under existing NRC programs will provide the data base necessary to validate models in containment codes so that meaningful full-scale containment calculations can be performed. Participants included representatives from NRC, DOE, the UK, national laboratories, industry (EPRI, FAl, PLG and various utilities) and universities.

Formal presentations and discussions took place in the following topic areas:

1. The Current Perspectives of Risk from DCH; 2. The Experimental Program; 3. The Analytical Program. The list of specific technical questions was previously provided to the participants.

From the discussions and deliberations, a consensus was reached that DCH was not dominant for the three plants studied in NUREG-1150 (Surry, Zion and Sequoyah) largely due to plant specific attributes. However, High Pressure i

Melt Ejection (HPME) and DCH could be dominant contributors to risk for other plants, and therefore could not be generically dismissed solely from 1150 results.

The general conclusions from the workshop were that:

-(1) The scaling rationale of severe accident phenomena is a key factor in setting up experimental programs and should continue to be pursued, (2) cavity and lower compartment configuration play a key role in the l-

dispersal characterization of the ejected melt, and need to be modeled in experimental test facilities, and (3) the key phenomenon that appears to be dominating the uncertainty associated with DCH loads is fragmentation and entrainment of the melt during the ejection process.

,The new Local Area Network (LAN) for Headquarters Operations Officers, together with' associated modifications to SINET, will be operational on January 8.

Event Notifications (50.72s) will be available on both old and new systems through January 12.

The Palo. Verde Diagnostic Team held its final meeting to compile findings. The 1

report is scheduled to be completed the end of January.

- Division;of Safety. Programs.(DSP) t AE00/T921, " Inadequate Capacity of 4160V Switchgear at FitzPatrick," was issued

-during the week of December 27, 1989. This report documents a potential problem at the FitzPatrick plant and the corrective actions being taken following our.

review of the problem.

i In May 1989, an NRC inspection team questioned the adequacy of the 4160V switch-gear capacity at FitzPatrick.

In July 1989, a licensee-calculation established that during the periods'that the unit's emergency diesel generators (EDGs) are tested in parallel with the plant normal station service transformer (NSST), the maximum fault current may excead the momentary and interrupting short circuit

. ratings of the 4160V switchgear. The licensee issued a JC0 for continuing this mode of operation, based on the low probability cf a 3-phase or phase-to-phase

-. fault, occurring during the 1-hour EDG monthly test. The probability figure by the licensee was based on a review of LER data base. The AEOD review of available operational data bases indicates that not all electrical faults are covered by LERs, and that even in LERs, faults are not reported in sufficient detail to identify all 3-phase and phase-to-phase faults. Also, the number of electrical faults identified by our search was.substantially more than those of the licensee.

Thus,:the probability approach used in the JC0 was questionable. Based on these findings, the licensee has agreed not to test EDGs in parallel with the NSST and-ensure that the 4160V switch gear is not exposed to fault currents beyond its rating. -

An AEOD member attended a regular meeting of the Nuclear Industry Check Valve Group (NIC) held on December 6 and 7,1989. There are three active committees

.in NIC:

the Technical Information Comittee (TIC), the Non-Intrusive Examination Comittee (NEC), and the Check Valve Application and Maintenance

-Practices Committee (CAMP). A trip report dated December 27, 1989 details the l organizations and ongoing activities of NIC.

NIC's efforts are promising in providing practical information to utilities to implement actions to improve check valve performance.

4 U.S. Ecology has selected a site near Butte, Nebraska in Boyd County as a potential site' for the Central Interstate 1.ow-Level Radioactive Waste Compact's

- LLW disposal facility. The Butte site was chosen from three~1ocations which were examined as potential sites. The site must be licensed by the State.

Meetings with.Rhode. Island and. Massachusetts. State. Liaison.0fficers Marie Miller, Region I State Liaison Officer, met with Charles Mansolillo, Director of the Governor's Office of Energy Assistance and Intergovernmental Relations-and Rhode Island State Liaison Officer, on January 3,1990 in Providence, Rhode Island. The purpose of the meeting was to discuss items of mutual interest, including the status of the Agreement State program, the LLW-disposal contract with the Rocky Mountain LLW Board and the status of NRC activities at Pilgrim and Seabrook.

In addition, Ms. Miller attended a hearing of the Connecticut Hazardous Waste Management Service Board to receive public L

comments on the 1989 update of the Connecticut LLRW Management Plan on the evening of January 3 in Danielson, Connecticut. -It was announced that the earliest a preferred site, a site designer and site technology could be selected would be June 1, 1991. On January 4,1990, Ms. Miller attended the NRC Pilgrim Restart Panel meeting with Boston Edison on the final assessment report for Pilgrim. Ms. Miller also met with' Buzz.Hausner, Massachusetts-State Liaison Officer designee, to discuts the status of the State inspection /

observation activities and to discuss the status of the Massachusetts LLW h

program.

Archived.E11es on Maonetic. Tape Approximately 1,000 NRC computer files which resided on the Brookhaven National Laboratory (BNL) computer system in 1985 were transferred to magnetic tape when NRC closed the computer service contract with BNL in

.early 1986. Many tapes.have not been used since that time. The.ITS Codes'and Stardards Section has maintained documentation of this tape library, most of which are stored at NRC or the Idaho National Engineering Laboratory (INEL) Archive Tape Library. A memorandum has been sent to each tape owner of record, asking that a tape control form be. filled out and returned to ITSB~to update our records.

CintichemcInc.

On December 12,1989..Cintichem, Inc., reported the discovery, on November 28, 1989, of slightly contaminated water in an onsite storm drain. A Regional-Inspector was sent to the site on December 19, 1989, to review the licensee's actions and analytical results. On the basis of that review, other actions taken by the licensee, and discussions with Region.1 management, the licensee issued a letter dated December 28, 1989 which confirmed commitments made with respect to the continuation of investigative and corrective actions to correct the identified onsite storm drain water contamination problem.

In addition, an informational

meeting was held at the Tuxedo, New York site on January 2,1990 to L

discuss potential sources of the contamination, actions taken to -verify -

and/or correct these sources, a plan of action for continued corrective

' measures, and identification of the skills and resources needed to accomplish the planned actions. This meeting was attended by licensee, NRC and New York State, Dept. of Labor and Dept.. of Environmental Conservation personnel. The licensee will continue to investigate the sources of the contamination. Review of recent analytical data indicated I

that there is still no evidence of offsite releases from the holding pond.

'The Regional Administrator and the Restart Panel met with representatives of.BECO on 1/4 at the Chiltonville Training Center to discuss the

' licensee's final assessment report.

Nine.M11e. Point. Unit.1 Niagara Mohawk. commenced reloading of the.Nine Mile Point Unit 1 core at p

approximately 11:00 p.m. on 1/3. Reloading was authorized by the Unit 1 Station Superintendent following approval of the temporary reload procedures and confirmation of system operability by the Operations Department. D. C.. Cook An enforcement conference was held in the Regional Office on January 4, 1990..with the American Electric Power Service Corporation to review the apparent violation resorted in NRC Inspection Report Nos. 50-315/89028(DRS);

50-316/89028(DRS). ' Tie apparent violation concerned anomalous readings 1

observed by the NRC inspectors associated with a flow switch for the Unit 2 turbine driven auxiliary feedwater pump (TDAFP) that affected the operability of the pump. The NRC staff outlined the circumstances of the NRC observation and the licensee provided additional information and the

- results of:further investigations into the safety significance of the problem and its corrective actions.

On December 14, 1989, an unanticipated turbine trip / reactor scram and subsequent reactor feedwater transient occurred on Unit I when the unit was at approximately 35% power. Region III-formed a special inspection team to investigate the event. The cause of the event was determined to be the installation of an incorrect turbine /feedwater pump trip switch during the refueling outage in November 1989.- The feedwater transient was an expected response for a turbine trip and it was bounded by the accident analysis. The licensee replaced the trip switch with the correct switch and commenced power escalation on December 15, 1989. 'A management meeting will be held with the licensee on January 11, 1990, in the Region III office to discuss the licensee's plans to improve the quality of work j

activities at Quad Cities Station.

The plan proposed by UPR will stabilize the material-inplace by covering the contaminated materials with native soil. The staff's technical position on cover design was also discussed.

'The purpose of the meeting was to discuss the staff Technical Position on Erosion Protection of Taiiings Piles-as it relates to Homestake's proposed reclamation plan for the Grants Mill, Homestake's amendment request for constructing a new evaporation pond, and Homestake's preliminary design for a new tailings disposal pond.

into Technical Specification surveillance for insoluble in the fuel oil for two of three diesel 1

generators (D/Gs)onsite. The-licensee believed that the problem was due to the method-of sampling (from the bottom of the fuel oil tanks) rather than bad fuel. Their belief that this was -the case was due to the fact that previous samples of the fuel tanks and fuel shipments ware tested satisfactorily.

In addition, testing using a newer method (sampling at theD/Gfilters)indicatedthatinsoluableswerewithinspecificationsand thir Supply System believed that the D/G's were operable. The licensee requested Technical Specification (TS) relief based on the above testing and the intention to revise the TS to delete the old testing method and replace it with the newer testing method.

On December 30, 1989, while starting up from an extended refueling outage, the Palo Verde Unit 3 phase "A" main transformer developed what appears to be an internal fault. The apparent fault caused excessive pressure within

the transformer, the spraying and draining of the internal oil, and the splitting of several seams in the transformer housing. The transformer produced smoke and an occasional flame and the fire was declared out by site personnel within ten minutes. There was danage to the phase "A" transformer components and a surge capacitor bank inside the turbine building. The reactor was at 12% power at the time of the event and was stabilized at 10% power afterwards. Safety-related equipment was not adversely affected by the event. The licensee intends to replace the transformer with a spare located on-site and perform a root cause evaluation. The licensee expects the transformer replacement to take E

The Commission requested the staff to:

Provide the Commission a copy of NUMARC's draft guidance to utilities on accident management and staff's comments on the draft.

2.

Submit an information paper which identifies those licensees requesting delays in submitting their IPEs, their justification for these requests, and staff's evaluation of the justification.

3.

Submit an information paper describing the candidate accident management strategies, prior to issuing them to

industry for information.

4.

Provide a status report on the Accident Management Program and proposed guidance to be provided to the industry.

5.

Develop the guidance and criteria, for Commission review and approval, which would outline in detail the role that the IPE program will have in assessing industry's status with respect to the Commissicn's Safety Goal Policy Statement.
