NRC INSPECTION OF SAN ON0FRE UNITS 1, 2, AND 3

This refers to the routine inspection conducted by Mr. G. Cicotte of this
office on October 16-20, October-30-November 3 and November 8,.1989
activities authorized by NRC License Nos. DPR-13, NPF-10, and NPF-15, of
, and to
=the discussion of our findings held by Mr. Cicotte with Mr. R. Krieger and
other members of your staff at the conclusion of the inspection.
Areas examined during this inspection are described in the enclosed inspection
report. Within these areas, the inspection consisted of selective
. examinations of-)rocedures and representative records, interviews with
personnel, and caservations by the inspector.
Based on the results of this inspection, it appears that certain of your
activities were not conducted in full compliance with NRC recuirements, as set
forth in the Notice of Violation, enclosed herewith as Appencix A.
In relation to the violation identified in Appendix A, we invite your
attention to 3aragraphs 2.A and 2.F of the enclosed inspection report.
The
findings in tie area of respiratory protective equipment maintenance tend to
indicate a lack of attention to detail, inadequate training and insufficient
supervisory oversight.
These findings might indicate a declining trend in
performance for the radiation protection program..
Additionally, subsequent to the inspection your staff p'rovided to us an audit
report of respiratory protection which concluded that respirators are
properly cleaned, maintained, selected and issued."
It is clear your self
assessment of this program did not present an accurate appraisal to senior
station management.
We urge you to evaluate whether this could be indicative
of a more general problem with your internal audit system.
Your response to this Notice is to be submitted in accordance with the
provisions of 10 CFR 2.201 as stated in Appendix A, Notice of Violation.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures
will be placed in the NRC Public Document Room.
