APPENDIX A NOTICE OF VIOLATION Southern California Edison Comapny Docket No. 50-361 As a result of an NRC inspection on March 13 to April 14, 1981, and its accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),

the following violation was identified:

Appendix B of 10 CFR 50, Criterion V, states, in part, that, " Activities affecting quality shall be prescribed by dccumented instructions, procedures, or drawings,...and sh611 be accomplished in accordance with these instructiors, procedures, or drawings."

SCE Operating Procedure 5023-3-2.6, " Shutdown Cooling," requires in part that valves HV-9339, HV-9337, HV-9377, and HV-9378 be opened to initiate shutdown cooling.

Contrary to the above,'the inspector determined that on March 21, 1981 shut-

down cooling had been initiated without opening valves HV-9377 or HV-9378.

the As a result of the above failure to follow an applicable procedure, Low Pressure Safety Injection Pump, P-016, was operated for at least several i

minutes with a combination of valves closed, such as to prevent a supply of wat'er to the pump suction.

This is a Severity Levei V violation (Supplement' II).

Pursuant to the provisions of 10 CFR 2.201 Southern California Edison Company is required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the 4

corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved, bn.!er tha authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted l

under oath or affirn.ation. Consideration may be given to extending your response time for c?od cause shown.
