h APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Millstone Unit 2 Hartford, Connecticut 06101 License No. DPR-65 Docket No. 50-336 As a result of the inspection conducted on January 1 through February 14, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified:

Technical Specification 6.8.1 states ii. part:

" Written procedures shall be established, implemented and maintained covering the activities referenced below:

a.

The applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, November 1972."

Regulatory Guide 1.33 dated November 3, 1972, Appendix A Paragraph C, states in part " Instructions for energizing, filling, venting, draining...

should be prepared,as appropriate, for the following systems:

1. Reactor Coolant System."

Station Procedtre OP 2301E, " Draining the Reactor Coolant System", Revision 6, dated 7/21/80, Paragraph 7.1.8 stated, "When RCS is dcained to the required level vent off the excess N2 to ambient pressure as follows:..

Close 2-RC-015 N2 to pressurizer."

Station Procedure OP 2301D, " Filling and Venting the Reactor Coolant System",

Revision 6, Change 2, dated 10/8/80, Valve Check Off List (0ps Form 23010-1) requires in part that valves 2-RC-015 and 2-RC-030 be shut during plant operation.

Contrary to the above, on January 6, 1981, during maintenance involving a pressurizer safety valve, reactor coolant pressure boundary valve 2-RC-015 l

was left open while the nitrogen header was isolated using the containment l

isolation valve 2-SI-312. Additionally, Reactor Coolant Pressure Boundary valves 2-RC-015 and 2-RC-030 remained open during a plant heatup to Operational Mode 3 on January 7,1981.

This is a Severity Level IV Violation (Supplement I).

1 Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company would be required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:

(1) admission or denial of the alleged violations; (2) the reasons for the item (s) of noncompliance if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective sceps which will be l

taken to avoid further violations; and (5) the date when full compliance will be achieved. However, your corrective action was taken during this 1

inspection period and has been described and verified in the attached i

inspection report. You have also documented your corrective actions in a letter to the NRC dated February 27, 1981.
