Technical Specification G.9.2.1.(9) requires prompt reporting of " Performance of structures, systems, or components that requires remedial action or corrective measures to prevent operation in a manner less conservative than assumed in the accident analyses in the safety analysis report or technical specifications bases; or discovery during plant life of conditions not specifically considered in the safety analysis report or technical speelfications that require remedial action or corrective measures to provent the existence or development of an unsafe condition."

Descciation of the Event:

During re-evaluation of concrete block walls in ecsponse to IC Dulletin 80-11, four walls (68.1 to 68.4) were identified which were unstable under the effects of tornado depressurization and cardox injection.

These are the computer room' walls in the cable spreading room.

Probable Consequences of the Occurrence:

Collapse of the walls could affect safety-related cables which pass through or are in close proximity to the walls.

Cause of the Event

Cause of event is design error.

Corrective Actions

On May 22, 1981, the computer room doors wero blocked open by installing angle stops.

This opening will prevent wall failure due to tornado d pressurization or cardox injection.

9 Permanent design modification is to install vents in the walls at each end of the computer room.

Our prompt notification on May 20, 1981, indicated that ducts would be required to raise the effective vent level, however, further review has determined that the computer room vents can be located such that air conditioning and cardox levels will be maintained without the ducts.

Permanent modifications will ba completed as soon as possible.
