This refers to the April 15, 1981 telephone conversation between Mr. Cooney of your staff and Mr. McCabe of this office regarding the April 1, 1981 discovery of three isolated drywell pressure transmitters at Peach Bottom Unit 2.
Two of the isolated pressure transmitters supply initiation signals to emergency core cooling systems and the third provides an input to the reactor protection system. Although automatic safety syster actuation was not prevented, redundancy was degraded. This event indicates a lack of operational control over safety-significant components. To prevent recurrence, we understand that you have committed to the following.
1.
Expeditious conduct of a Unit 2 valve lineup check for all safety system actuation instruments, to include all instrument root valves. Valves located in radiation areas of one Roentgen per hour or greater will be checked during the next unit shutdown. The Plant Operations Review Committee will review this effort for adequacy.
2.
Development and approval of detailed Unit 2 and Unit 3 valve lineup check-lists which provide for independent verification of proper lineup of the above instruments.
3.
Conduct of a lineup using the approved checklists on Unit 2 as soon as practicable and on Unit 3 prior to Startup from the current refueling outage.
