Divis-Besse Unit 1 INDENTIFICATICN OF OCCURRENCE:

Unsealed half-inch holes through fire walls Conditions Prior to Occurrence: The unit was in Mode 1, with Power (Mfr) = 2042.9 and Load (Gross ME) = 684 Description of Octmrrence: On March 16,1981 at 1300 -hours, the NRC Site Inspector -

reported finding four h inch unsealed holes through the Control Rocm Equignent Focm east fire wall. The holes open into the stairway east of the 03ntrol Ibcm and appear to have been left by the rcmoval of a plate frca the wall. The holes were Wintely filled with Kav.w3ol.

On April 3, 1981 at 1440 hours0.0167 days <br />0.4 hours <br />0.00238 weeks <br />5.4792e-4 months <br />, the NFC Site Inspector found two 15 inch unsealed loles in the north wall of rocm 427B, Ioa Voltage Switchgear Room. A firewatch was es-tablished until the holes were packed with Kaowool at 1535 hours0.0178 days <br />0.426 hours <br />0.00254 weeks <br />5.840675e-4 months <br />.

In each case, the Station had entered the action statenent of Ibchnical Specification 3.7.10.

The Technical Specification requires all penetration fire barriers protect-ing safety related areas to in functional at all times. The action statement re-quires that with a penetration fire barrier inoperable, establish a continuous fire watch on at least one side of the affected penetration within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />. Both of these incidents were written up on DVR's and sulraitted April 3,1981.

Designation of Apparent Cause of Occurrence: The cause of this was personnel error as the holes should have been filled with Kaowool when they were first opened and the proper people should have been informed so the appropriate actions could have been taken to log the holes in the penetration log and to have the holes permanently filled with grout.

Analysis of Occurrence:

There was no danger to the health and safety of the public l

or to unit personnel. There was no fire which required the fire wall to have its integrity intact.

Corrective Action

The holes were ir:frediately filled with Kaowool and under Main-tenance Work Order's 81-1837 and 81-2200, the holes were permanently filled with I

grout.

Failure Data:

Previous violations of penetration fire barriers were reported in NP-33-80-11 (80-008), NP-33-79-63 (79-060), NP-33-79-61 (79-059), and NP-33-78-137 (78-117).
