Narrative

Report for LER 50-366/1981-025.

On April 3,1981, with the reactor at 96% power, a loss of Secondary Containment occurred on Unit I. This placed a Limiting Condition for Operation on Unit 11 per Tech Specs Section 3.6.5-1, which states that Unit I must maintain Secondary Containment when Unit II is at power.

A similar event was reported on Reportable Occurrence No. 50-366/1980-142.

Contract personnel were performing required activities for the installation of the Torus Clean-up System.

Secondary Containment was inadvertently broken as a pipe penetration was drilled from the Turbine Building into the Reactor Building. A Thaxton Plug was used to seal the penetration and Section "G" of HNP-1-3653, Stand-by Gas Treatmeat Operability Test, was performed satisfactory and Secondary. Containment was proven operable at 1245 on April 3, 1981.

The generic review did not reveal any inherent problems.

However, to prevent an event of this nature from re-occurring, personnel have been instructed on the proper methods for core drilling when the possibility of breaking Secondary Containment exists.

Public health and safety were not affected by this incident.
