This placed the unit in the action statement of Technical Specification 3.6.3.1, Containment Isolation Valves, which states that CV5011A must be operable with a maximum isolation time of fifteen seconds. Pursuant to this action statement, CV5011A was immediately shut using the hand operator and its breaker was de-energized to ensure the valve remained in its safety position.

Designation of Apparent Cause of Occurrence:

Under Maintenance Work Order 81-1889, the operator for CV5011A was inspected.

It was discovered that one tripper was There are two trippers and cams badly worn and was not riding on its tripper cam.

With one disengaged from its cam, the operator could not reliably shift from hand operation to motor operation. The tripper had been installed in the proper manner but was sliE tly out of adjustment which caused the wear.
