' Report No. 50-210/74/ 21 IDENTIFICATION Violation of the Technical Specifications, Table 3.1.1.B.2, OF OCCURRENCE:
which specifies that the RE22 main steam line high flow sen-sors actuate at a differential pressure corresponding to a line flow of <120'. of rated steam flow (97.5 psid).
This event is considered to be an abnormal occurrence as de-fined in the Technical Specifications, paragraph 1.15B 6 G.
CONDITIONS PRIOR TO OCCURRENCE:
X Steady Stato Power Routine Shutdown Hot Standby
~ Operation Cold Shutdown Load Changes During Refueling Shutdown Routine Power Operation Routine Startup Other (Specify)
Operation The major plant parameters at the time of the event were as follows:
Power:
Core,1750 MWt Elec., S95 MWe 5
Flow:
Recirc., 15.1 x 10 gpm Feed., 6.46 x 10' lb/hr Stack Ges: 27,000 pCi/sec
,s DESCRIPTION At 0040 on March 13,19 /4, while taking daily Technical Speci-0F OCCURRENCE:
fication log readings, an operator neted and reported to the Shift Foreman that the differential pressure indicator for Sensor RE22D indicated 0 psid and that the instrument lines were hot. The operator found the RE22D bypass valve to be open, causing Sensor RE22D to be inoperabic.
Upon closing the bypass e e

The apparent cause of this incident is under investigation.
ANALYSIS OF The safety significance of this occurrence is minimal as 7 of OCCURRENCE:
the 8 high steam flow sensors remained operable.
In the event of a high steam flow, 3 sensors were available to sense a high I
flow condition on Main Steam Line 1, and 4 sensors were avail-able to detect a high flow in Main Steam Line 2.
CORRECTIVE Recommendations will be made pending evaluation of the incident ACTION:
