Fast Insertion Pulser of the Nuclear Instrumentation

Conditions Prior to occurrence _:

Prior to and during the time of this occurrence, the reactor was operating at power levels of 55-95t j
with three loops and six primary coolant pumps in service.

.De_scription of occurrence:
On several occasions during the pe'.iod of this i
occurrence, the electrical feed to the Past Insertion I
Pulser was interrupted due to failure of the power l
supply and/or blowing of protective fuses.

Description of Appa_ rent cause of occurrence:
continuing investigation has not yet indicated l
any apparent cause for this problem.
8.
Analysis of Occurrence:
As indicated in earlier reports dealing with A.O. 4-1-8, which also involved the loss of the 3
Past Insertion Pulser, the safety implications of this occurrence are slight.
9.
Corre_ctive Action:
Immediate corrective 'a'ctions have, been to replace -

the power supply and/or fuses, as necessary, which re-l turns.the pulser to service.
Final corrective action will be based on the outcome of the continuing inves-j tigation of this problem.
