O APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company License Nos. NPF-4 North Anna NPF-7 Based on the NRC inspection August 21 - September 30, 1980, certain of your activities were apparently not conducted in full compliance with NRC require-ments as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Technical Specification 6.8.1.a, written procedures shall be established implemented, and maintained for activities as recommended in Appendix "A" of Regulatory Guide 1.33 Revision 2, February 1978. Regulatory Guide 1.33, Appendix "A" specifies in Paragraph 3, procedures for the operation of ECCS and Containment Cooling systems.
Contrary to the above, Valve 1-QS-61 in the Quench Spray and RWST systems was positioned shut during the period July 21 to August 21, 1980 although valve lineup procedure 1-0P-7.7A requires the valve be open and no procedure exists requiring the valve be shut.
This is an infraction for Unit 1 only. A similar item was brought to your attention in our letter dated August 20, 1980.
B.
As required by Technical Specification 6.5.1.6d, the Station Nuclear Safety and Operating Committee (SNSOC) shall review all proposed changes or modifica-tions to plant systems or equipment that affect nuclear safety.
Contrary to the above, the SNSOC reviewed Jumper #173 two (2) days af ter the modification was installed.

This is an infraction for Unit 2 only. A similar item was brought to your attention in our letter of March 26, 1980.
C.
As required by Technical Specification 6.8.2, applicable procedures recom-mended in Appendix "A" of Regulatory Guide 1.33 and changes thereto shall be reviewed by the SNSOC and approved by the Station Manager prior to implementation. Appendix "A" of Regulatory Guide 1.33 Paragraph 9 specifies maintenance procedures that can affect the performance of safety-related equipment.
Maintenance procedure MMP-C-EG-1, the approved procedure for maintenance of the emergency diesel generator engines directs the technician, in Steps 5.1 and 5.3, to disassemble and reassemble, according to the procedures in the Colt Industries Service Manual.
l Contrary to the above, Colt Industries Service Manual for Model 3800 TD8-1/8 diesel engine procedures for disassembly and reassembly of the emergency diesel generator engines, safety-related equipment, were not reviewed by the SNSOC or approved by the Station Manager. Further, no controls exist to insure that the service manual used by the technician is complete and up to date.
This is an infraction for both units.
