At 0851 hours0.00985 days <br />0.236 hours <br />0.00141 weeks <br />3.238055e-4 months <br /> on September 20, 1980, with the plant in Operational Mode 5, the Motor-Driven Fire Pump (FP-P-1] was started for a surveillance

  • test.

The Diesel-Driven Fire Pump (FP-P-2].was on clearance for piping modifications. At 1429 hours0.0165 days <br />0.397 hours <br />0.00236 weeks <br />5.437345e-4 months <br />, while (FP-P-1] was being shutdown after completion of the test, the pump was discovered running with oil running down the motor housing. The motor was smoking *and a small pool of oil was on the floor. The pump was immediately shut down.

Investigation revealed that cooling water had been lost to the upper motor bearing cooler. The valve lineup was verified to be correct.

A temporary fire pump was verified to be operational, this pump supplies 40% of the fire system design flow.

At 2050 hours0.0237 days <br />0.569 hours <br />0.00339 weeks <br />7.80025e-4 months <br />, [FP-P-2] was returned to service but the modification remained to be completed. An additional temporary pump has been made available and the modification completed.

The loss of cooling water to the motor bearing cooler was caused by debris lodged in the cooling water isolation valve upstream of the strainer.

When the fire pump was started as per the surveillance procedure, the cooling water flow was verified. During the pump run, a large piece of debris became lodged in the cooling water isolation valve'and, thus, stopped cooling water flow. This occurrence is considered to be random in nature.

The cooling water isolation valve has been cleared. The motor has not at this time been repaired. This is the second occurrence of this type (see Special Report dated March 5, 1980). The testing procedure requires that the couling water flow be ensured. No further corrective action is deemed necessary.
