Dear Mr. Warner:
This letter addresses the inspection of your facility at Cincinnati, Ohio, conducted by Messrs K. R. heidu and H. M. Wescott, of this office on Septenter 12-15, 1989, and the discussions of their findings with Mr. D. Selke and other nembers of your staff at the conclusion of the inspection.
The purpose of the inspection was to verify the implementation of your quality dssurance program in selecttd areas and observe activities related to the manuf acture, assembly, and testing of pneumatic-hydraulic valve actuators intended for the Hope Creek nuclear power plant.
Areas examined during the NRC inspection and our findings are discussed in the-enclosed report. This inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.
During this inspection, the inspectors observed that the implementation of your QA program failed to meet certain NRC requirements.
Specifically, the inspectors determined that your quality assurance program was inadequate in the areas of describing the authority and duties of persons performing safety-related activities; measures to convey quality requirements contained in customer pur-chase orders to subvendors; measures to verify the adequacy and validity of documentary evidence related to purchased material; measures to document in-process inspections on components manufactured; measures to control com-ponents and material that do not conform to specifications; and measures to control the calibration of equipnient.
Furthermore, the inspectors determined that you do not have a procedure to implement 10 CfR Part 21 and you had not posted the documents required by 10 CFR Part 21. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
