APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company.
Docket No. 50-245 Hartford, Connecticut 06101 Based on the results of an NRC inspection conducted on October 27-31, 1980, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC license as indicated below.
This item is a deficiency.
Technical Specification 6.8.1 states, in part: " Written procedures shall be established, implemented and maintained...."
Technical Specification 6.8.2 states, in part: "Each procedure... change [ ]
...shall be reviewed by the PORC...and approved by the Unit Superintendent...
prior to implementation...."
Operations Department Instruction 1-0PS-1.03, "Special Work Assignments,"
assigns responsibility to ensure that valve lineup checkoff lists, data sheets, and other forms established by procedures that are kept in the Control Room Forms Cabinet be maintained current to the latest revision.
Contrary to the above, certain procedures were not properly implemented; valve lineup checkoff lists and other forms in the Control Room Forms Cabinet were not maintained current to the latest revision; and, a procedure change was implemented without getting proper review and approval in that:
Form OP207-1, Revision 0, dated November 15, 1977, Scram Report Data Sheet, was performed July 12, 1980, although Revision 1, dated April 16, 1980 was in effect; Checkoff List (COL) 321-1, Service Water System Valve Checkoff List, Revision 1, dated August 23, 1978, was performed June 24,1979 although Revision 2, April 12,1979 was in effect; COL 333-1, Service Air System Valve Checkoff List, Revision 0, was being maintained in the Control Room Forms Cabinet although interim change No.1 to Revision 0, dated September 18, 1980, was in effect; Fom OP 209-2, Procedure Required for Shutdown, Revision 1, plus interim change 1, was being maintained in the Control Room forms cabinet although Revision 2, dated September 18, 1980, was in effect;
- and, Procedure OP 302, paragraph 7.11, Control rod drive stall flow testing, was performed on June 26, 1980; July 24, 1980; August 25, 1980; and September 18, 1980 recording data on a data sheet other than that specified in the procedure without obtaining a procedure change which was reviewed by the PORC and approved by the Unit Superintendent.
