Dear Mr. Grier:
On behalf of the Eastman Kodak Company, I affirm to the follow-ing in response to your letter to me of January 20, 1981, your Docket No. 70-1703:
In reference to the last paragraph on page 1:

Corrective steps which have been taken -
On January 16, 1981, E.
Scott Harter, Radiation Safety Officer of the Eastman Kodak Company, made application (copy attached) to the Nuclear Regulatory Commission through Mr.
William Crow for a six-month temporary license for the Special Nuclear Material covered by our lapsed License No. SNM-1513.

Corrective steps to be taken to avoid this happening in the future -
The permanent file for this license has been kept in the office of the Company Radiation Safety Officer.
A duplicate file will be established in the Office of the Director of the Analytical Science.s Division and will incorporate a revolving l
index of this, and other, licenses to alert us to the need to file renewal applications or execute other periodic responsibili-ties associated with these licenses.
This file will serve as a l
backup to that maintained by the Company Radiation Safety Officer.
In reference to the first paragraph on page 2:

I received your letter of January 20, 1981 at approxi-l l
mately 1:30 PM on that date.
In accordance with the instructions given to us by MA. J.
Roth on January 16, 1981, I notified the group leader of tl:e Neutron Activation Facility that I had been ordered to shut down the Californium Flux Multiplier (CFX) on the completion of experiments currently in progress.
At 4:30 PM on January 21, 1981, I took possession of the operating key for the CFX and gave the group leader, Mr. J. L. Pietruszewski, a wit-nessed and notarized receipt for the key.
I then had the key placed in a safe provided for such purposes by the Administrative Services group of these Laboratories.

The security of the SNM was assured by the physical barriers and administrative procedures described in License SNM-1513, all of which I affirm to be fully in place.
(C)
At 2:00 PM on January 21, 1981, I was informed that Mr.
Crow had notified Mr. Harter of the approval of our temporary license and that we could resume operation at any time.
I then reversed the process described in (A) above and returned the operating key to Mr. Pietruszewski.
Receipts for these trans-actions were also witnessed and notarized.
At Mr. Roth's suggestion, I enclose with this letter copies of each of the receipts for the transfer of the key which I affirm to be exact copies.
I appreciate the help that Mr. Roth gave me in our telephone conversation of January 22, 1981 in formulating this response.

Dear Mr. Crow:
Eastman Kodak Company requests that License No. SNM-1513 be reinstated for a six-month period, effective on today's date.
A revised application will be submitted within one month.
This request has been discussed with Dr. An-Liang Soong of your staff during his visit to our facility today.
Please note that a renewal request was submitted on June 16, 1980, and the application fee ($460) was submitted in August 1980.
Efforts to expedite this request will be greatly appreciated since a costly shutdown of our operation is in effect.
