FINAL REPORT Description of The Deficiency A review of test-in-progress documentation for a pneumatic pressure test conducted on portions of the service air system revealed an item of noncompliance. The item involved failure to conduct the test in accordan,ce with written, approved procedures. This item is a recurrence of test control deficiencies identified by the Nuclear Regulatory Commission Office of Inspection and Enforcement (NRC-0IE) on the Reactor Coolant System (Inspection Report Nos. 50-327/80-43 and 50-328/80-21) and the Tennessee Valley Authority's (TVA's) subsequent commitment (letter from L. M. Mills to J. P. O'Reilly dated November 12, 1980) to ensure the adequacy of the conduct of preoperational and related tests in accordance with written approved procedures. This item represents a breakdown in TVA's QA program and the ability to implement that program.

While the service air system is considered a nonsafety-related system, the portions of the system being tested were identified as TVA Class B piping and would therefore be considered safety related. The portions of the service air system identified are at penetrations in the steel containment vessel.

Safety Implications Since this condition had occurred previously on the reactor coolant system and is repeated on the service air system, it conceivably could have occurred on another safety-related system. If this were to occur, the ability of that system to function properly could be affected.

s Interim Corrective Actions The completed test data for the suoject pneumatic pressure test conducted on portions of the service air system has been reviewed and approved.

TVA has taken action to ensure the adequacy of the conduct of future construction tests by issuing a memorandum from the construction engineer to management supervisory personnel responsible for conducting construction tests. The memorandum clarifies recent commitments made to the NRC and instructs responsible test personnel to implement existing procedures and ensure that:

All test activities are performed using approved procedures.

2.

All test activities are reviewed by responsible individuals or units.

3 All tests are conducted by a qualified test director who is capable of ensuring that all procedural requirements are implemented.

TVA has taken additional action by issuing Interdivisional Quality Assurance Procedure, (ID-QAP) 11.2, " Construction Test Control." This procedure was issued March 9,1981. It defines organizational functions and responsibilities and establishes divisional interface in support of the construction test program.
