l 80-21 (A) When Vital Bus IV lost power, due to the inverter breaker l opening, the control for "A" S/G failed open. This diverted feed flow from "B" and "C" S/G's. "C" S/G Level went to the Lo Lo Level Reactor Trip Set Point before the "A" S/G Hi Level turbine trip could be actuated.
80-22 (H) S/G Level Control was in manual during the startup.
80-23 (A) A suspected tube failure in the 6B feedwater heater had occurred requiring removal of the heater from service.
When cooling water was valved out, the wat.cr volume in the shell side expanded, causing a high level automatic furbine Trip.

UNIT SHUTDOWN AND POWER REDUCTIONS EXPLANATION SHEET DOCKET NO. 50339 REPORT MONTH _ September UNIT NAME N. Anna 2 YEAR 1980 DATE 10-80
, COMPLETED BY A. G. Neufer (703) 894-5151 ext. 221 80-05 A While in the process of cleaning a condensate pump suction strainer one of the strainer valves'was not fully shut oue to debris under the valve seat. This caused a low vacuum condition tripping the turbine resulting in a reactor trip.
