Linear Indications in Gib Gusset Welds in Reactor Vessel Structural Support (Neutron Shield Tank)(Your let*ar of August 14,1980)

Thank you for your letter, referenced above, which famarded an interim report pursuant to 10 CFR 50.55(e) regarding the subject matter.

This matter was reviewed by our inspector August 11-15, 1980.

In addition, during inspection 50-412/80-06, our inspector reviewed your inspection and repair activities in connection with the significant deficiency reported December 7,1979 for the skirt to base plate weld of the neutron shield tank.

Our inspection report 50-412/80-06 foentified our concerns regarding that defic-1ency and your corrective actions. These concerns included a question about the effectiveness of the fabricator's quality control program and the S&W procurement surveillance program. Our recent review of this matter and the number and extent of the linear indications identified in conjunction with the deficiency reported earlier increases our previously expressed concern about the OC and procurement surveillance programs.

Additionally, it brings into question the quality of welding performance; including workmanship, welding procedures and possibly joint design.

In view of the above, we request that you inform us of yout actions in regard to the following:

determination of the extent of the deficiencies in all gib gusset welds; examination of all other welds on this structure not previously inspec*ad onsite; determination of the causes of all deficiencies identified; and, corrective actions and preventive actions to prevent recurrence of similar deficiencies.
