Gentlemen:

Subject: Report Nos. 50-327/80-44 and 328/80-23

This refers to your letter of March 4,1981, informing us of steps you have taken

to correct the violations concerning activities under Operation License No.

DPR-77 and Construction Permit No. CPPR-73 brought to your attention in our

letter of February 6,

1981.

We have rev'ewed these responses and noted the

following inadequacies.

Your response to item A (328/80-23-01), did not address the corrective actions

taken by TVA to determine the extent of hydrostatic testing deficiencies, and the

causes of and corrective actior.s for these other deficiencies.

Your response to item B (327/80 *4-01, 328/80-23-02), did not address the need

for corrective actions in the adjustment of motor operated valves which were

adjusted prior to the implementation of Revision 3 to SNP Inspection Instruction

No. 24.

Your response to item C (327/80-44-02, 328/80-23-03), implied that previous

standard practices for flushing instrument sensing lines were adequate though not

documented.

From a discussion with your engineering personnel, our inspector

.

noted that the standard practice was- not adequate.

In addition to lacking

acceptance criteria, the standard practice was not coordinated with process line

flushing. We consider it inappropriate to introduce flushing effluents into a

previously certified clean system.

It is our understanding that in a telephone conversation of March 24, 1981,

between 0. Ormsby of TVA and J. Mcdonald of this office, it was agreed that TVA

would supply supplemental information to these responses. We will examine your

corrective actions and plans during subsequent inspections.
