Docket *10. 50-a60 Construction Per-it No. CPPR 13a SPPF' DIX A NOTICE OF VIOLATI0il As a result of the inscection corducted between flovember 17 and December 5, 1980 and in accordance with the Interin Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

10 CFR 50, accendix B, Criterion V. as imolemented by Section 17.1.5 of the cuality assurance procram documented in the PSAR and the WPPSS QA Progran Manual Procedure OAP-2, Paraaraoh 3.1, states, in part, that " Activities affecting cuality shall be Drescribed by documented instructions, orocedures or drawings, of a type appropriate to the circumstances and snall be accomplisned in accordance with these instructions, crocedures or drawings...."
A.1 Detail drawina lio. CK37847, detail 3, issued to J. A. Jones Construction Cenpany oy United Engineers and Constructors regarding the installation of nuclear service water heat exchangers, an activity affecting quality, specifies that Washers are to be installed between the anchor bolt nuts and the baseplates of the heat excnangers.
Contrary to tne acove, on !;ovember 17, 1980, none of the 32 anchor bolts used for attaching the nuclear service water heat exchangers l A and 23 to their fcundations had washers installed between the nut and baseplate.
The torcuing of ti.e anchor bolt nuts had been cornleted and verified by J. A. Jores inspection personnel on Februa ry 14, 1930.
A.2 J. A. Jones Construction Company procedure fio. POP-N-50lW (Use of Process Control Sheets) specifies in paragraph 5.3 that " mandatory hold points shall be sianed off by Quality Verification" and further specifies in caragraph 6.5 that " Work shall not proceed past the hold point until Quality Verification signoff of the process control sheet."
Contrary to the above, as of flovember 18, 1980, work had proceeded past three hold points without Quality Verification signoff on J. A. Jones process control sheet flo. JAJ-MI-0278.
This process control sheet was used for control of the installation of nuclear service water heat exchanger rio. 2B, a quality affecting activity.
This is a severitv level V violation (Supplement II of Federal Register flotice 45 FR 66754, dated October 7,1980).

10 CFR 50, Appendix B, Criterion V, as implemented by Section 17.1.5 of the quality assurance program cocumented in the PSAR and the WPPSS OA Progran Manual Procedure OAP-2 Paragraph 3.1 states, in part, that " Activities affecting quality shall be prescribed by documented instructions procedures and drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings.
Instructions, procedures or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Table 3.2-2 of the PSAR specifies that the containment spray system piping will meet the requirements of the ASME Boiler and Pressure Vessel Code for Section III, Class 2 piping.
Subsubarticles t1C2660 and itC4240 of the ASME Boiler and Pressure Vessel Code for Section III, Class 2 pioing specifies requirements for welded joints, includina weld dimension and geometry.
Contrary to the above, as of fiovember 18, 1980, 3/4-inch diameter threadoiet fittings had been welded to the 10-inch diameter containment spray piping and the joining welds were inspected and accepted by the contractor althougn the quantitative acceptance criteria of the ASME Code for weld dimensions and geometry were not included in the instructions, procedures or drawings used to control the work.
Installation of these fittings was specified by United Engineer and Constructor's field change notice flo. 211-1-80-113, Revision 1 and isometr'. drawing ilo. 412443, Revision 2.

Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply i
System is hereby required to submit to this office within twenty-five days of the date of this i;otice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
