O APPENDIX A NOTICE OF VIOLATION Georgia Power Company License Nos. DPR-57 Hatch NPF-5 Based on the NRC inspection September 13 - October 14, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Technical Specification 6.8.1, written procedures shall be established, implemented and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33.
Regulatory Guide 1.33, Appendix A, states in part that procedures should be provided which control radioactivity and limits personnel radiation - exposure.
Plant Procedure HNP-009, Personnel contamination survey, Paragraph G.I. states that "... all oersonnel contamination should be immediately reported to health physics."
Contrary to the above, two workers were observed on October 8,1980, leaving a hand and foot monitor after it alarmed. The workers decontaminatadtheir hands without notifying health physics.
This is an infraction. A similar item was brought to your attention in our letter dated September 12, 1980.
B.

Regulatory Guide 1.33, Appendix A, states in part that "... procedurer should be provided which control radioactivity and limits personnel radir' ion exposure."

Plant Procedure HNP-8005, Paragraph E.3.b., requires that radioactive labels be initialed and dated by the technician performing the survey.
Contrary to the above, an HP technician updated three tags on September 12, 1980, and failed to initial or date the tags.

Plant Procedure 8108, Paragraph F.4. requires that HP supply another dosimeter when an employee finds his off-scale.
Contrary to the above, an HP technician on October 9,1980, failed to provide another dosimeter to an employee who found his off-scale.
3.
Plant Procedure HNP-8004, Paragraph G.3. requires that HP document the re-zeroing of a dosimeter on a Form 3, Dosimeter Re-zero Record.
Contrary to the above, an HP technician on October 9,1980, failed to document the re-zeroing of an off-scale dosimeter on a Form 3.
These items constitute an infraction.
