1-28-81 This event occurred because a spring loaded pin on the mechanical slip-on connection between the flow paddle and transmitter of the flow indicator /

recorder became loose due to vibration in the ef fluent discharge line causing an inaccurate flow signal to be sent to the recorder. The slip-on connection was reassembled with a new pin, the flow indicator / recorder and transmitter were checked for calibration and system was returned to normal operation.
