NOTICE OF DEVIATION I
i Based on the results of an NRC inspection conducted on September 29 - October 3, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.
j Criterion V of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings.
Instructions, pro-i cedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Deviations from these requirements are as follows:
A.
Stone & kebster's River Bend Project Procedure RBP 2.11-1-1 dated September 5,1980, paragraph 3.1.2.3, states in part, "Other reviewers shall be selected by the lead materials engineer in j
accordance with EAP 4.13, paragraph 3.2.3.
The specification and all addenda / revisions must be reviewed and approved by Construction l
and QA."
l Contrary to the above, Specification No. 211.161, revision 3 dated June 28, 1980, (Specification for Nonengineered Items) was not i
approved by either Construction or QA.

Stone & Webster's Engineering Assurance Procedure EAP 5.3, 1
-Revision 3, dated February 8, 1980, Attachment 1.2, states in part' "When a form is used for the Calculation Title Page l
that does not contain a separate Independent Reviewer signature block, the Independent Reviewer shall place an "I" after his/her name to indicate completion of the review of methods and assumptions or shall place an "I/NI" after his/her name to indicate completion of the entire review (i.e., the Independent I
and Non-Independent reviews)."

2 Contrary to the above, on Pipe Stress Analysis Sumaary - Low Pressure Core Spray,12210-AX-78C-2, Issue 2, the form used did not have an Independent Reviewer signature block and the Independent Reviewer did not place an "I" after his name.
Corrective action and preventive measures were taken concerning the above i
findings prior to completion of the inspection, therefore no written response to these findings is necessary.
