iY APPENDIX A NOTICE OF VIOLATION Sacramento Municipal Utility District Docket No. 50-312 Based on the results of an NRC inspection conducted on June 9-13, 1980 it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your Facility License No. DPR-54 as indicated below.
Items A and B are Infractions.
A.
10 CFR 19.12. " Instructions to Workers", states in part, that all individuals working in or frequenting any portion of a restricted area shall be instructed of their responsibility to report promptly to the licensee any condition which may lead to or cause a violation of Comission regulations and licenses or unnecessary exposure to radiation or to radioactive materials, and shall also be instructed in the appropriate response to warnings made in the event of any unusual occurrence or malfunction that may involve exposure to radiation or radioactive material.
Contrary to this requirement, on June 9 and 11, 1980 individuals were provided training which permitted them unescorted access to the restricted area without being instructed in their responsibility to report promptly to the licensee any condition which may lead to or cause a violation of Commission regulations and licenses or unnecessary exposure to radiation or to radioactive materials and they were not instructed in the appropriate response to the emergency evacuation amber strobe lights located in high noise areas of the facility.
B.
Technical Specification 6.4, " Training" states that:
"A retraining and replacement training program for the operating staff shall be maintained under the direction of the Training Supervisor and shall meet or exceed the requirements and recommendations of Section 5.5 of ANSI N18.1-1971 and Appendix "A" of 10 CFR Part 55."
Rancho Seco Administrative Procedure AP700, " Rancho Seco Training Program" developed pursuant to this requirement
'in Enclosure 4.9, " Chemistry / Radiation Division Training Program" describes the initial or replacement training progran for these individuals and includes the following statement regarding retraining:
"Every two (2) years employees shall complete a program consisting of chemistry procedure qualificatio.ns; equipment checkouts; and chemistry and health physics procedure review.
The Chem / Rad Supervisor shall prepare this retraining program which shall be reviewed and approved by the Station Training Supervisor."
Contrary to this requirement, as of June 9,1980 the retraining program for members of the Chemistry / Radiation Division has not been maintained in that the Chem / Rad Supervisor has not prepared a two year retraining program for review and approval of the Training Supervisor.
