Dear Mr. O'Reilly:
On February 7, 1980 a report was made under the provisions of 10CFR50.55(e) concerning nonconformances identified during our review of I.E.Bulletin 79-14. On February 21 an' February 28 supplemental reports, identified herein as supplements 1 and 2, respectively, were made on additional nonconformances under the provisions of 10CFR50.55(e).
The items identified in the original 10CFR50.55(e) report were judged as not requiring reporting under 10CFR21; however, the items in supplements 1 and 2 were reported under 10CFR21 on February 28, 1980.
In accordance with the reporting requirements of 10CFR21, the following information is submitted:

Description of defect, deficiency, or failure to comply:
In our valve and operator weight verification program required by I.E.Bulletin 79-14, we have identified nonconformances in the form of overstressed p1;ms, pipe supports, and equipment nozzles resulting from charjes in valve and operator weights.
The attached Table I provides t'ie details of each nonconformance; the nonconformances are grouped by MSK (an MSX is an isometric piping drawing).
