WPPSS NUCLEAR PROJECTS 3 AND 5 NRC INSPECTION OF WNP-3 AND WNP-5 DOCKET NUMBERS 50-508 AND 50-509

Reference:

1). Letter, G.S. Spencer to F.D. McElwee, NRC Inspection at WNP-3 AND WNP-5, dated January 9,1981.

This letter is in response to your letter of January 9,1981 which discussed the results of the inspection conducted December 8-19, 1980 of activities authorized by Nuclear Regulatory Commission Construction Permit Numbers CPPR-154 and CPPR-155. The letter identified one item of noncompliance and required the Supply System to provide a response to this item.

The specific Nuclear Regulatory Commission Finding, as stated in your letter, and the supply System response is provided in Attachment I to this letter.

ATTACHMENT I NUCLEAR REGULATORY COM.9IS$'ON FINDING:

As a result of the inspection conducted on Decerber 8 to 19,1980, and in accordance with the Interim Enforcement Policy, 45 FR56750 (October 7,1980),

the following violation was identified:

10 CFR 50, Appendix 3. Criterien IX, states, in part, that, " Measures shall be established to assure that special processes, including, welding, ...are controlled and accocplished by qualified cersonnel using qualified procedures in accordance with applicaole coces..." ,

Paragraph 17.4.9 of the Quality Assurance Program documented in i the PSAR states, in part, tnat "...Pe. 'ncel performing special processes '

at the ccnstructicn site will be qualified as required by the applicable ccdes and standards..."

Construction contract no. 3240-232 specifies that the AWS Dl.1 Structural Walding Code is the applicable code for structural welding, including qualification. Figure 5.27 of the AWS code identifies the configuration of the guided-bend test jig to be used in testing welcer qualification test coupons. The figure specifies that for material with a yield strength of less than 50,000 PSI the plunger and interior die radii shall be 3/4" and 1-3/16" respectively.

Contrary to the above, on Decer,ber 16, 1980, it was fcund that the site HVAC contractor (contract 3240-232) has been qualifying welders using an improperly sized guided-bend test jig. Welders who have been r.aking qualification test coupens with ASTM A-36 caterial (yield strength r,f approximately 36,000 PSI) have been qualified based on testing with a jig naving plunger and interior die radii of 1" and 1-7/16", respar.tiveiy.

I This is a Severity Level IV violation.

Corrective Stecs Which Have Been Taken And The Results Achieved.

Wallace/ Superior issued Site Nonconformance Report #2853 fated December 17, 1950 for Class I welding and ceased all welder qualification act'vities. The Supply Systea imposed on Wallace/ Superior (letter G03-60-3207 dated December 17,1980)

Stop Work Order #232-02. This stop work order was issued to ensure tr -t corrective action was implemented prior to the resumption of Class I work.

The Contractor identified, as required by the NCR, those welders qualified with  !

the questioned guided bend test equipment and the corresponding Class I producticn welds perforced by those individuals. 'he welders were requalified to the requirements of AWS DI.1. The production welds had been inspected and 3:cepted to the requirements of AWS D1.1. Based on the above actions, the production Class I welds were accepted.

Corrective Steps Which Have Been Taken To Avoid Further Items of Noncompliance:

The guided-bend test jig has been segregated to prevent inadvertant use.

A new guided-bend test jig has been requisitioned by Wallace/ Superior.

The Date When Full Compliance Will Be Achieved:

The rew guided bend test jig arrived at the site in February 1981. Wallace/

Superior has performed receipt inspection of this fixture, and has confirmed conformance with applicable AWS Dl.1 requirements. The Engineer has also con-firmed dimensional conformance with appropriate AWs D1~.1 requirements. However, Stop Work Order 4232-02 documented additional deficiencies outside the bounds of this specific violation. The Stop Work Order will be rescinded when all deficiencies have been resolved satisfactorily.

The position stated in Wallace/ Superior's letter, 232WP-81-0176, dated December 18, 1980, regarding its interpretation of the configuration requirements of the guided-bend test jig as set forth in Figure 5.27 of AWS D.l. has since been confirmed as being valid by the American Welding Society. Therefore,Wallace/

Superior is preparing a response to WPPSS concerning this issue and may request that this violation be recinded.
