NOTICE OF VIOLATION s

As a result of the inspection conducted on December 8 to 19,19P' and in accordance with the Interin Enforcement Policy, 45 FR66754 (Oct er 7, 1980),

the following violation was identified:

10 CFR 50, Appendix B, Criterien IX, states, in part, that, " Measures shall be established to assure that soecial processes, including, weiding,

...are contralleo and accomplished by _oualified personnel using qualified precedures ',n accorcance with applicable codes...."

Paragraph 17.4.9 of the Quality Assurance Prooran documented in the PSAR states, in part, tnat "... Personnel performing special processes at the construction site will be qualified as required by the applicable ccdes and stancarcs...."

Construction contract no. 3240-232 specifies that the AWS D1.1 Structural Velding Coce is the applicable code for structural welding, including qualification.

Figure 5.27 of the AWS code identifies the configuration of the guided-bend test jig to be used in testing welder qualification test coucons. The figure specifies that for material with a yield strength of less than 50,000 PSI the plunger and interior die radii shall be 3/4" and 1-3/16" respectively.

Centrary to the above, on Decencer 16, 1980, it was found that the site HVAC contractor (contract 3240-232) has been qualifying welders using an improperly sized guided-bend test jig. Melders who have been making qualification test coupons with AST!! A-36 raterial (yield strength of apprcxinately 35,C00 PSI) have been qualif ted based on testing with a l

jig having plunger and interior die radii of 1" and 1-7/16", respectively.

This is a Severity Level IV violation (Supplement II).

?ursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply-Systen is hereby required to submit to this office within twenty-five days of.the date of this flotice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of nonccmpliance; and (3) the date when full compliance will be achieved.
