APPENDIX A NOTICE OF VIOL \\ TION Tennessee Valley Authority License No. DPR-77 Sequoyah Unit 1 Based on the NRC inspection August 1-31, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Technical Specifications, Section 3.6.3.1, containment isolation valve FCV-61-192 shall be operable with a maximum isolation time of less than or equal tc ten (10) seconds with the plant in Modes 1, 2, 3 or 4.
Contrary to the above, on July 27, 1980, containment isolation valve FCV-61-192 was identified to require approximately 20 seconds to stroke closed with the plant in Mode 3.
No action was taken to comply with the Limiting Condition for Operation of the action statement of Technical Specifica-
- ion 3.6.3.1.
This is an infraction.
B.
As required by Technical Specifications, Section 6.8.1, written procedures shall be established, implemented and maintained covering the applicable procedures in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Appendix A of Regulatory Guide 1.33 requires that instructions for startup, shutdown, and changing modes of operation be implemented for the emergency power sources systems.
Contrary to the above, on August 9, 1980, failure to follow the System Operating Instruction for resetting the emergency diesel generators following an automatic start, resulted in the non-essential trip inhibit relays for all four emergency diesel generators being made inoperable for approximately 24 minutes.
In this condition, the non-essential diesel trips would not have been bypassed upon a safety injection signal as required by Technical Specification 4.8.1.1.2.C.7.C.
This is an infraction.
