LICENSEE EVENT REPORT NARRATIVE DESCRIPTION

Dear Mr. Grier:

The following was reported to Mr. C. Cowgill, Region I, Office of Inspection and Enforcement on January 14, 1981.

Technical Specification 3.5.F.1 requires that "During any period when one Diesel Generator is inoperable continued reactor operation is permissable....provided that... containment cooling subsystem...shall be operable.

Descriotion of the Event While performing a diesel generator full load surveillance test with Unit 2 at 100% power, the E-3 diesel generator output breaker failed to close and tie the generator to the E-32 4kv emergency bus .

1 Probable Consequence of the Occurrence At the time of the failure, the 2B containment subsystem was out of service. As required by Tech. Spec. 3.5.F.1, an orderly shutdowr; was initiated . The failed breaker was promptly replaced with a spare unit, tested satisfactorily and the shutdown was

, terminated. All ot:~.er ECCS equipment was available and the time 4

period for which the problem existed was short, therefore, safety significance was minimal.

Cause of the Event:

A misaligned interlock switch was the cause of the problem.

This switch functions to prevent the breaker from closing when it is not fully in place.

Corrective Actions:

The breaker was replaced with a spare unit and successfully tested. All other 4 kv emergency bus feeder breakers were tested and fo und to be satisfactory. The defective switch was realigned.
