This event is considered to be a t e si.able occurrence as defined in the Tachnical F4 F4 cations, paragraph 6.9.2.b. (3).

Conditions Prior to Occi.u.i=ce 21e plant was operating at steady state power until about 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br /> on Novenber 21, 1980 at which time a reactor shutdown was ccmnenced for reasons unrelated to this event. On Monday, nacs=hal 15, 1980, during a review of the Master Survai11anca Schedule it was discovered that the Diesel Generator Ltnthly Bat 9_ry Survaillanca (Plant Procedure 636.2.006) and Main Statica Mcnthly Battery Survaillanca (Plant Procedure 634.2.003) were not performed as scheduled for the ucnth of Novenber.

The tests were scheduled to be pe&f-=d en Neverber 21, 1980. Die ::enthly battery checks were previously Muu=1 on Cui.ei.+c 24, 1980 but not done again until Decenber 19, 1980. During a review of the recards it was also ennfim=ri that the ucnthly battery checks were also not pm.few =1 within the required interval in Aucust 1980.

'Ihe manufacturer's instructions for battery care rm-4 that written records be kept to follow the performance of batteries. 'Ihis type of survaillance has been dennstrated through avnariance to provide an indication of a cell hamming irregular or unserviceable long before it t+2 - em a failure. The batteries will deteriorate with time, but precipitous failure is unlikely. 'Ihe Tec.5u'ical Apaci*ications required that the voltage of each battery cell be measured nonthly. 'Ibe weekly battery tests, which were peifw.u=:d as scheduled, require the measurement of the pilot cell voltage and total battery voltage. A review of the data recorded on the weekly tests indicated no obvious dam =dation in the overall condition of the batteries. In addition, the monthly battery checks performed in the follow 2ng acnths indicated all the individual cell voltages were within acceptable limits.

Corrective Action

'Ihe electrical department reviewed the bidant with their personnel and stressed the importance of pm.fw.uu.ng all - W survaillanca testing within the Tari#ied intervals. In addition, the department has inplemented practices to ensure the situation does not recur. 'Ihese practices include:

A copy of the survaillanca schadnla will be maintained and signed cff by the aes w. late supervisor when receiving the a:npleted documenc.

The appropriate supervisor will review the survaillanca testing require-ments due for the week in advance with the person responsible to perform the test.

3.

An end of the week verification by the appuriate supervisor will be pw1&asizi to assure c:Epletion of all scheduled surveillance tests.
