This letters refers to the telephone conversation between Mr. W. Johnson, Vice President, Yankee Atomic Electric Company, and Mr. L. E. Tripp of this office on December 22, 1980 concerning the repetitive items of noncompliance associated with weld repairs in safety-related piping systems. The specific concerns are described in the reports of three separate NRC inspections conducted in April, September and November of this year. Each inspection resulted in items of noncompliance in the area of weld repairs which were performed by the piping subcontractor (Pullman-Higgins). Based on the above conversation, it is our understanding that you have accomplished, or plan to accomplish, the following:

1.

Immediately issue a stop work order to halt all weld repairs perfonned by Pullman-Higgins.

2.

Establish measures to assure that correct welding procedure specifi-cation and inspection criteria are (1) specified prior to repair of welding, (2) verified to have been properly accomplished, and (3) checked for applicability to actual repair conditions after completion.

3.

Provide more comprehensive training to appropriate Pullman-Higgins personnel in the criteria associated with the welding procedure specification applicability and NDE requirements for repair welding.

4.

Increase licensee quality assurance involvement in the audit and actual witnessing of repair welding activities.

In addition to these specific actions, we understand that you will take the following longer tenn generic actions:

Establish generic measures to assure that licensee controls including surveillance and audits are sufficient to avoid repeated recurrence of nonconfonning activities.

6.

Provide a means of assuring that corrective actions to NRC inspection and internal audit findings are broadened to include the generic aspects of the findings and not merely the specific finding.

We understand that with respect to Items 1-4 above, notification will be made to the Region I Office by telephone and in writing describing your specific actions and that Region I concurrence with your specific corrective actions applicable to repair welding will be obtained prior to the resumption of such welding by Pullman-Higgins. Your written response to Items 1-4 above will include plans and schedules for the implementation of the longer term actions required by Items 5 and 6.
