APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Based on the results of an NRC inspection conducted on August 25-28, 1980, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Facility License, DPR-61, as indicated below.
A.
10CFR50.54(1-1) states, in part, " Holders of operating licenses in effect on September 17, 1973 shall implement an operator requalification program which, as a minimum, meets the requirements of Appendix A of 10 CFR 55...
which was submitted for approval by the Atomic Energy Commission."
The Haddam Neck Plant Operator Requalification Program dated December 21, 1977 states in Section 4.4 that, 'No more than two licensed personnel will be exempt from evaluation based on their participation in the preparation and administration of the evaluations."
Contrary to the above, three licensed personnel were exempt from the 1979 Annual Requalification Program Examination.
This is a deficiency.

10 CFR 55, Appendix A states, in part, "3.
On-the-job training.
The requalifcation program shall include on-the-job training so that:

Each licensed operator and senior operator reviews the contents of all...
emergency procedures on a regularly scheduled basis."
The Haddam Neck Plant Operator Requalification Program dated December 21, 1977 states in Section 2.2 that, "All Emergency Procedures will be scheduled for annual review."
Contrary to the above, no evidence was available to document that all operators had reviewed all emergency procedures during 1979.
This is a deficiency.
