CAUSE DESCR.PTION AND CCR AECflVE ACTIONS l

I s ic t IInvestigation revealed that the loss of feedwater flow was due to a failes i,liiipump shaft.

The pump failure was caused by heat stress due to a failure,

i,i: l I of the radial & thrust bearinas.

The root cause of the bearing failures l i,;3: l is being investigated.

The pump rotating assembly was rebuilt and the i

tiuilDumo returned to operable status.
