Narrative

Report for LER 50-366/1980-163 On Friday, 12-12-80, Bechtel Power Corporation notified the plant site that there was a potential' problem with Unit 2

reactor head insulation structure in that it may not meet seismic class I requirements.

This came about during review of pictures taken at the site of i

the insulation for design of the r e f u e ling bellows insula-tion.

On 12-13/14-80, the site reviewed the drawings and information from Bechtel, and on Monday, 12-15-80, the site notified Bechtel of the results of the investigation.

Bechtel then confirmed that indeed the insulation structure did not meet seismic class I requirements and that the structure would need modifications as originally proposed by.Bechtel during the Unit 2 construction phase.

The insulation structure will be modified before Unit 2

startup scheduled in early February.

This condition does not affect Unit

1. which has a similar insulation because Unit I has been verified to have already been modified to conform to seismic class I requirements.

l There was no effect on public health and safety as a result of this event.

of the event was an oversight in i

modifying the insulation structure during construction of Unit 2.

No other cause or details of this oversight is available or known to exist at this time.

No further corrective action other than that stated above is planned or deemed necessary.
