s Appendix A NOTICE OF VIOLATION Docket No. 50-10 Commonwealth, Edison Docket No. 50-237 Company Docket No. 50-249 Based on the inspection conducted on March 2, 3, 7, 8, 14-16, 22, 23, 27 and 28, 1978, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 and 2 are infractions.
1.
Unit-1 Technical Specifications, Section 3.6.A.1, requires that the average rate of reactor coolant temperature during normal heatup or cooldown not exceed 100 F/hr when averaged over a one hour period.
Contrary to the above, during a startup of Unit-1 on January 21, 1978, the 100 F/ hour heatup rate was exceeded as a result of inadequate procedures and contributing personnel error. The licensee's corrective action to prevent reoccurrence has been reviewed and appears adequate therefore, no further response is required.

10 CFR 50, Appendix B, Criterion 5 requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, and shall be accomplished in accord-ance with these instructions, procedures, or drawings. The licensee's Quality Assurance Manual and Administrative Procedures (QP 14-51 and DAP 3-5) require out-of-service equipment to have affixed to it 00S cards. DAP 7-2, " Conducting of Operating Shif t Routine," requires that at a minimum the items to be covered during a shift turnover include the status of the unit systems and problems.encount2 red during the concluding shift and their solutions or the progress made towards solving them.
Contrary to the above, the licensee failed to affix an out-of-service card to the gland seal leakoff drain pump for the Unit 2 HPCI system when that pump was taken out of service on March 22, 1978, and failed to turnover the status of the gland seal leakoff drain pump during the shif t tornover between the midnight and day shifts.
