O Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-10 Based on the inspection conducted on October 19-21 and 25, 1977, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. The items are infrrctions.

Contrary to Technical Specification 6.2.D, requiring two members of plant management, at least one of whom helds a Senior Reactor Operator's License, approve temporary procedure changes, the licensee failed during September, 1977 to properly make temporary changes to one surveillance procedure, DOS 500-4, and six operating procedures; DOP 020-M2, DOP 240-M1, DCC 240-M2, DOP 1000-M1, DOP 3500-M1, and DOP 5400-M2.

Infraction 'A:
Contrary to Technical Specification 6.2.A, the licensee failed during September, 1977, to adhere to an approved procedure in that there were no Shift Engineer's reviews following the com-pletion of two cperating procedures, DGP l-1 and DOP 5400-M4, and the Surveillance Procedure DOS 300-5.
1 Discussion:
During the course of this inspection, the inspector identified three instances of a lack of proper Shift Engineer These three review of Operating and Surveillance Procedures.
procedures were completed along with many others which were adequately reviewed during the Unit 1 startup in September, 1977.
The Shift Engineer review of the above three procedures was inadvertently omitted.
Corrective Action and Corrective Action to Prevent Recurrence:
Etation management realizes the importance of proper review of completed procedures.
In order to emphasize the buportance o* proper review, all Shift Supervisors will be informed in wl' ting that the station received an item of non-compliance resu' ting from inadequate procedure review and that future reviews must be improved.
