Infraction 1:
Contrary to Technical Specification 6.2.D, requiring two members of plant management, at least one of whom holds a Senior Reactor Operator's License, approve temporary procedure changes, the licensee failed during September, 1977, to properly make temporary changen to one surveillance procedure, DOS 500-4 and six operating procedures; DOP 020-M2, DOP 240-M1, DOP 240-M2, DOP 1000-M1, DOP 3500-M1, and DOP 5400-M2.
Discussion:
During the course of this inspection, the inspector identified deficiencies in one completed Operating Department Surveillane2 (DOS 500-4) and six completed Operating Department valve checklists.
The deficiency identified with DOS 500-4 involve' step F.ll of tne procedure.
This step appears in the procedure as follows:

When the proper results have been obtained, the test is complete.
Return valve MO-169 and MO-170 to the desired position and STOP the mechanical vacuum pump and the auxiliary oil pump.
NOTE When reactor startup is in progress, the above valves / pumps shall remain open/ running.
In the procedure completed 9/20/77, the Nuclear Station Operator (NSO) lined out the words "STOP the mechanical vacuum pump and the auxiliary oil pump" to indicate that these pumps were not stopped.
However, since a reactor startup was in progress at the time the surveillance was completed, the station be;tieves that the NSO action was within the limits permitted by the note following step F ll. and that corrective action is not necessary on this item.

The deficiency identified with the six completed operating valve checklists involved leaving valves in positions other than those specified in the checklist.
The purpose of a valve checklist is to inform the Shift Supervisors of valve positions in the plant and to determine if the valve positions are proper with respect to plant conditions.
While conducting the six valve checklists listed above, the plant operations shift determined that plant conditions would not permit certain valves to be positioned as specified in the checklist.
Corrective Action and Corrective Action to Prevent Recurrence:
The station believes the plant operations shift supervision should have the authority to approve changes to electrical and mechanical checklists by having one licensed Shift Cupervisor initial the change in the margin.
The appropriate Drecdon Administration Procedures will be revised to reflect this policy.
